多层螺旋CT对肺动静脉瘘的诊断价值(附2例报告并文献复习)

【摘要】 目的 探讨多层螺旋CT对肺动静脉瘘的诊断价值。方法 利用16层Aquilion多层螺旋CT扫描机对2例肺动静脉瘘进行容积增强扫描,并行肺部血管重建(MPR ,MIP ,VR等影像后处理技术)。结果 增强扫描横断面图像示肺内病灶强化程度与肺动脉分支类同,连续的MPR重建图像可显示病灶与肺血管相延续,肺部血管MIP及VR重建图像可清晰显示肺动静脉瘘的影像特征———供血动脉、动脉囊瘤及引流静脉,多角度和多方向的旋转观察能显示三者关系的细节。结论 多层螺旋CT肺部血管成像是诊断肺静动脉瘘的首选的非侵袭性影像方法

【Abstract】 Objective To study diagnosing value for pulmonary arteriovenous malformations with multi-detector row CT. Methods Two cases of pulmonary arteriovenous malformations were examined with 16 slices multi-detector row CT. The pulmonary vessels were regenerated by multi post-processing technology such as MPR,MIP and VR. Results The contrast enhanced axial images showed the enhanced density of lesions similar to that of pulmonary arterial branches. The communication of the lesion and the pulmonary vessels was demonstrated in the consecutive images of multiplanar reconstruction. The reconstructed images of pulmonary vessels with regenerated MIP and VR can detect imaging characteristics of PAVMs-supplied artery, draining vein and cystic aneurysm. Three-dimensional reconstructed vessel imaging with multi-angle and multi-direction can show more detailed relation among the three structure l. Conclusion Multi-detector row CT angiography of lung is the first selective non-invasive imaging method for diagnosing pulmonary arteriovenous malformations.
