家兔角膜上皮植片无饲养层构建与自体移植

【摘要】 目的采用无饲养层细胞培养体系构建家兔角膜上皮组织,自体移植后观察其对角膜缘干细胞缺失的治疗效果。方法将14只家免制作成角膜缘于细胞缺失的病理模型。用角膜缘组织块或体外分离2~4代的角膜缘干细胞接种到去除上皮的人羊膜基底膜上培养,待培养3~4周上皮细胞长成复层后,7只家兔角膜缘干细胞缺失的病理模型自体移植构建的羊膜角膜上皮植片,另7只家兔病理模型作为对照组,其中4只仅移植羊膜,另3只不做移植。观察其重建角膜上皮层的移植效果。结果经临床观察和组织学检查,与仅移植羊膜和不做移植的家免相比,7只自体移植构建的角膜上皮植片的家兔,角膜的透明度增加,新生血管明显减少,效果明显。结论采用无饲养层细胞培养体系构建自体角膜上皮组织,移植后可以成功修复角膜缘干细胞缺失的角膜上皮。为临床上由于角膜缘干细胞缺失所致的角膜疾患的治疗提供新措施。
【Abstract】 Objective To reconstruct rabbit corneal epithelium in vitro with a free-feeding layer culture system and autograft to treat limbal stem cells deficiency. Method 14 rabbits of total corneal limbal stem cells deficiency pathological model have been created. The corneal epithelium sheet have been reconstructed by cultivated limbal explant or 2-4th passage limbal stem cells on the denuded human amniotic membrane .when corneal epithelial cells have formed multilayers structure after 3-4 weeks , 7 rabbits received autologous corneal epithelial sheet, and 4 rabbits were only transplanted with denuded amniotic membrane ,the other 3 rabbits were not transplanted as control. Results After transplantation, followed by 3-9 months, compared with the cornea transplanted with amniotic membrane and nontreated cornea, the 7 autografted rabbits corneal epithelium have been successfully reconstructed by examined with histological methods. These corneal transparencies increased and neovasculavization decreased to a certain extent. Conclusion the reconstructed rabbit corneal epithelium hi vitro with a free-feeding layer culture system could be autografted to repair total corneal limbal stem cells deficiency. This is a new clinically approach to treat total corneal limbal stem cells deficiency.
