【摘要】 目的:采用多普勒组织成像(DTI)方法定量观察培哚普利(perinsopril)对老年人陈旧性心肌梗塞患者心功能的影响。方法:80例患者随机分成两组,试验组为培哚普利(4mg,qd,40例)与消心痛(10mg,tid)联用,对照组为消心痛(10mg,tid)单用组。疗程为16周。治疗前后患者均行常规二维超声(2-DE)及DTI检查,采用DTI重点测定二尖瓣环(MVR)收缩期运动速度(MVR-Vs)、舒张早期运动速度(MVR-DeV)、舒张晚期运动速度(MVR-DaV)(速度单位为m/s),结合M型超声测定左心功能方法测出左室射血分数(EF)。结果:试验组治疗前后MVR-Vs为0.076±0.014和0.087±0.013(P<0.01);MVR-DeV为0.079±0.017和0.088±0.016(P<0.05);MVR-DaV为0.095±0.014和0.104±0.014(P<0.01),以及EF为0.542±0.048和O.681±0.054(P<0.01)。对照组EF治疗前后为0.534±0.043和0.579±0.042(P<0.01),而DTI观察指标治疗前后对比无统计学差异。结论:DTI可定量分析左室壁心肌收缩与舒张功能为临床观察药物疗效、合理选择药物及预后判断提供了定量指标。更多还原

【Abstract】 Objective:To observe the influence of perindopril to cardiac function of the aging with old cardiac infection quantitatively by Doppler tissue imaging. Methods:There 80 cases which are divided into two groups randomly. Combination use of perindo pril(4mg qd 40 cases)and vasocardin(10mg tid)in test group. And only vasocardin(10mg tid)in contrast group. The period of treatment is 16 weeks. The patients had the examination of 2-dimensional ultrasound and DTI routinely before and after treatment. The mital vavular ring systolic velocity(MVR-Vs), eady diastalic velocity(MVR-DeV)and end-diastolic velocity(MVR-DaV)were analysed by DTI and the left ventricular ejecten fraction(EF)was measwed by M-mode ultrasound. Results:The values before and after treatment in test group are as fallows:MVR-Vs:0.076±0.014 and 0.087±0.013(P < 0.01); MVR-DeV; 0.079± 0.017 and 0.088±0.016(P <0.01);MVR-DaV:0.095±0.014 and 0.104± 0.014(P < 0.01)EF:0.542±0.048 and 0.681±0.054(P < 0.01)and the EF in contrast group before and after treatment is:0.534±0.043 and 0.579±0042(P < 0.01). But there is no statistic difference between before and after treatment by DTI observation. Conclusion:DTI can analyse quantitively the systolic and diastic caidiac function of left ventricle and provide quantitive standards fox clinical drug effects observation, selection of drugs and prognosis of treatment.更多还原
