【摘要】 目的探讨壁性成釉细胞瘤(mural ameloblastoma,MA)的组织来源,病理特点及X线特征。材料和方法收集术后病理确诊的MA12例,对病理诊断为成釉细胞瘤或牙源性颌骨囊肿的41例加切蜡块复查。结果复查41例中有9例符合MA诊断标准。21例MA中来源于角化囊肿19例,含牙囊肿1例,牙源性腺样囊肿1例。X线表现为:下颌升支5例,升支一体部15例,上颌窦区1例;单房11例,多房9例,房室大小接近8例,房差悬殊1例;囊腔呈圆形1例,轻度膨胀(短径(长径1/2)10例,局部膨胀8例,无膨胀2例;边缘出现分叶10例,切迹5例,光滑整齐7例;含牙3例,无牙18例,牙根吸收呈锯齿状8例,截根状9例,斜面状1例。结论:MA除主要来源于牙源性角化囊肿外,还可来源于含牙褒肿或牙源性腺样囊肿。MA主要的X线特点是:下颌升支和或体部单房或大小接近的多房影像,囊腔沿颌骨长轴生长,发生轻度膨胀或局部明显膨胀,边缘常见分叶或切迹,牙根锯齿状或截根状吸收明显。更多还原

【Abstract】 Objective To investigate the origin of tissue, the pathological and X-ray features of the mural ameloblastoma (MA).Method Collecting 12 post-operative cases diagnosed as MA and re-examining 41 cases diagnosed as ameloblastoma or odontogenic cyst of jaw by paraffin tissue section.Result Of the 41 cases re-examined, 9 meet the diagnostic criteria of MA. Of the 21 cases of MA, 19 are from the keratocyst, 1 from dentigerous cyst and 1 from odontogenic adenoid cyst. X-ray shows: there are 5 cases in the ramus of the low jaw, 15 in its body, 1 in the sinus maxillaries; 1 leases are the monoloculu, 9 are the multiloculi (8 with similar size, 1 with the big difference between the loculus); 1 with round loculus, 10 with slight expansion (short diameter less than the 1/2 of the long diameter), 8 with obvious partial expansion, 2 without expansion; 10 with the marginal lobule, 5 with the indentation, 7 with the smooth and ordered margin; 3 with the tooth, 18 without tooth, 8 with the absorption of tooth like the indentation, 9 like amputation, 1 like slope.Conclusion MA is mainly from the dentigerous and the odontogenic adenoid cyst besides the odontogenic kerotocyst. The main X-ray features of the MA include the image of the monoloculu or multiloculi with similar size in the ramus and /or body of the mandible which grows along the axis of mandible, with slight or partial expansion in the loculus; the lobule or indentation are often seen in the partial margin; there is obvious absorption like indentation or amputation in the tooth root.更多还原
