【摘要】 目的:研究儿童腮腺区血管瘤诊断和治疗方法。方法:回顾性分析四川大学华西口腔医学院于1990年5月-2000年12 月治疗的儿童腮腺区血管瘤病例。其中男性4例,女性7例。根据患者的临床表现、穿刺、超声波检查结果而作出诊断。经血常规正常,确定全身无其它严重疾患者,患儿在年龄6月后接受平阳霉素(pingyangmycin,PYM)瘤内注射治疗。把8mg 注射用盐酸平阳霉素溶解于8ml 生理盐水,并加入1nd(5mg)地塞米松制成注射药剂。根据 B 超的提示,决定穿刺深度,在回抽有血液,确定注射针在瘤体内后,注射平阳霉素药液,每次平阳霉素的用药剂量约1mg,1次/月,连续注射5次后,可间隔2-3月。用药总量一般不超过20mg,最大35mg。结果:11例患儿接受了 PYM 瘤内局部注射治疗,均获得了满意的疗效,瘤体消失,但局部皮肤有赘生表现。未发现本组患儿有任何不良发应或生长抑制现象。结论:平阳霉素瘤内注射疗法可作为儿童腮腺区血管瘤首选治疗方法,超声波检查对其诊断和治疗有辅助作用。更多还原

【Abstract】 OBJECTIVE:To investigate the diagnosis and treatment of hemangiomas in parotid gland of early childhood. METHODS:11 cases(4 males and 7 females)of hemangiomas in parotid gland were studied retrospectively,who had been treated in the clinic of the Stomatology Hospital of Sichuan University during the 11-year period from May 1990 to December 2000.These hemangiomas underwent the therapy of Pingyangmycin intratumorous injection,and the reagent was administered as a solution of 8mg in 8ml saline with mixing 5mg dexamethasone.The total dose of Pingyangmycin ranged from 5 rag to 35 mg,and was administered 1 mg per month.RESULTS:The curative rate was good,and the lesions were almost disappeared.There were no serious side effects of this treatment,such as pulmonary fibrosis and growth inhibition.On the other hand,ultrasonography was obvious valuable in the diagnosis and treatment of the lesion.CONCLUSION:Injection of Pingyangmycin maybe is a selective primary therapy method of the hemangioma in the parotid gland,and it can significantly reduce the size of the lesion mass.Ultrasonography should be useful for diagnosis and treatment of this lesion.更多还原
