单核/巨噬细胞参与血管瘤血管构成

【摘要】 目的:血管瘤是血管源性良性肿瘤,常见于儿童。尽管血管瘤的发病率很高,且可导致严重的并发症,其发病机制目前尚不清楚。本研究旨在探讨单核/巨噬细胞是否促进血管瘤的增长。方法:用免疫组化回顾性研究血管瘤和脉管畸形,检测病变中 CD68、CD34和 Ki67的表达。从人外周血中分离单核/巨噬细胞,用含 bFGF 和 VEGF 的培养基进行培养,细胞培养两周后免疫组化检测细胞表型。结果:不同时期的血管瘤中可见 CD68的表达,退化期血管瘤中 CD68阳性细胞明显减少,部分血管瘤内皮细胞可以共表达 Ki67和 CD68。血管畸形中未检测到 CD68的表达。在含有 bFGF 和 VEGF 的培养基中培养两周后,单核/巨噬细胞可以共表达 Glut-1,Fcγ/RII,CD34,UEA 和 vWF。结论:单核/巨噬细胞可以分化为血管瘤内皮细胞参与血管瘤血管构成。

【Abstract】 OBJECTIVE:Hemangiomas are highly prevalent in newborns and infants,leading to severe complications.However, the pathogenesis of hemangiomas is still unknown.This study was designed to examine the potential role of human macrophages/ monocytes to promote growth of hemangioma.METHODS:In a retrospective study of a variety of hemangiomas and vascular anomalies,we assessed lesional immunoreactivities for CD68,CD34 and Ki67 by immunochemistry.Purified monocytes/macrophages from adult human peripheral blood were cultured under a conditional culture environment supplemented with basic fibroblast growth factor(bFGF)and vascular endothelial growth factor(VEGF).Cells cultured for two weeks were subjected to histological and immunochemical examinations in order to determine the expression of specific markers for hemangioma endothelial cells.RESULTS: CD68-positive cells were found in all three phase of hemangiomas and the number of CD68+cells decreased dramatically in nonprohferative lesions.It was demonstrated that some hemangioma endothelium co-expressed Ki67 and CD68.No CD68 immunoreactivity was found in any vascular malformations(7 arteriovenous,33 venous,and 3 lymphatic)from patients aged 2 years to 65 years.Monocytes/macrophages cultured for two weeks in angiogenic medium expressed human erythrocyte-type glucose transporter protein(Glut-1),FcγRII and several other endothelial markers,all of which are deemed specific markers for hemangioma endothelial cells.CONCLUSION:Our data suggested that monocytes/macrophages are capable of differentiating into hemangioma endothelial cells and contribute to the vasculature in hemangioma.
