【摘要】 目的:初步探讨平阳霉素(pingyangmycin,PYM)治疗婴幼儿唇部血管瘤的安全有效方法,评价其疗效。方法:采用平阳霉素瘤内注射治疗婴幼儿唇部血管瘤9例,隔周1次,重复2～3次为1疗程,治疗后记录患儿的全身和局部不良反应,追踪随访, 观察临床疗效。结果:9例患儿全部治愈,其中注射1次4例,注射2次4例,注射3次1例。最初治疗的3例均出现局部溃疡, 改进注射方法和剂量后,后6例无1例发生溃疡。观察1～2．5a,发生溃疡部位未见瘢痕形成,无1例复发。注射后局部反应还包括肿胀。全身不良反应包括低热4例,食欲下降2例。无过敏病例。结论:平阳霉素瘤内注射治疗婴幼儿唇部血管瘤治愈率高, 疗程较短。但应用中需注意注射剂量,避免发生局部溃疡。更多还原

【Abstract】 PURPOSE:To investigate safe and effective method to treatment of hemangioma located in lips of infants with pingyangmycin(PYM)sclerotherapy.And evaluate its clinical efficacy.METHODS:9 cases were treated by intralesional injection of PYM.If necessary,the subsequent injections were performed fortnightly.A period of treatment was from 2 to 3 injections.The general and local adverse responses were recorded and the clinical outcomes were assessed.RESULTS:Complete clinical resolution acquired in all patients.In this series,4 cases received only a single injection and 4 patients received 2 injections.The remaining one patient received 3 injections.3 initial patients found overlying mucosa necrosis.After adjusted the injected dose,the following 6 patients found no necrosis any more.No clinical recurrence and scar were observed during a follow-up from 1 to 2.5 years.The transient local swelling occurred in almost all patients.The general complications included transient pyrexia in 4 cases and poor appetite in 2 cases. No allergy was found.CONCLUSION:Treatment of hemangioma in lip of infants by PYM sclerotherapy shows a high rate of complete resolution and a short therapy time.And a lesser dose of sclerosant is suggested to avoid post-injective necrosis.更多还原
