颌下腺转位预防放疗导致的口干症

【摘要】 <正>目的:评价颌下腺转位于颏下间隙后腺体功能及预放头颈部放疗导致的口干的效果。材料和方法: 实验组12例,根据Seikaly术式行颌下腺转位术,转位颌下腺13侧,术后接受全颈放射治疗,颈部放疗时适当屏蔽位于颏下区已转位的颌下腺。放疗后随访期最长44个月。对照组7例,术后接受全颈放射治疗,放疗后随访期最长15个月。术前术后检测颌下腺唾液流量,并填写口干调查表。结果:放疗后3个月,实验组静息状态和酸刺激状态颌下腺唾液流量都已经超越术前水平,至放疗

【Abstract】 Objective To evaluate the postoperative function of the submandibular gland which was transferred from submandibular space to submental space, to observe whether this surgical procedure could prevent radiation-induced xerostomia. Materials and Methods 14 submandibular glands were surgically transferred into the submental space before radiotherapy in 13 patients according Seikly-Jha procedure. Appropriate shielding was used to keep the transferred submandibular gland outside of the radiation fields. There were 7 patients in the control group. The patients were followed clinically, with saliva flow study and questionnaire of dry mouth. The following up interval was up to 44 months after radiotherapy in the experiment group and 15 months in the control group. Results After radiotherapy, the transferred gland still worked. The saliva flow in the third month after radiotherapy was more than those postoperatively. The patients of experiment group did not complain of dry mouth. While the saliva flow of the control group patients was decreased dramatically, the patients had severe dry mouth. Conclusion Surgical transferred of the submandibular gland to the submental space can preserve its function and prevent radial xerostomia.
