可滑动椎弓根螺钉框架手术治疗生长发育期脊柱畸形

【摘要】 目的:研究一种可滑动椎弓根螺钉框架结构,后路手术治疗生长发育期脊柱畸形, 防止因全程框架锁定固定影响脊柱生长而导致医源性躯干短缩、强直、旋转畸形。方法:设计一种经后路双侧椎弓根进椎体钉、棒、连杆框架。双棒矫形,中段锁定,两端可滑动,不影响脊柱纵向生长和椎体横向发育。结果:本组病例98例,术前平均弯曲66.43°,术后平均21.21°,矫正率 68%,术后6~36个月随访47例,纵向生长55.4cm,平均生长1.2cm。结论:可滑动椎弓根螺钉框架结构后路手术治疗生长发育期脊柱畸形,此结构具有坚强的三维矫形力量,矫形度数大,近期效果满意,脊柱纵向生长不受限制,无脊柱强直、旋转畸形和曲轴现象发生。
【Abstract】 Objective:A new kind of slidable pedical screw frame was designed to treat spinal de- formity in growth period Iatrogenic short trunk,stiffness and rotation deformity caused by long spine fixation with usual locked pedicle screw frame can be avoided.Method:This frame was composed of different type of pedicle screw,transverse link and rod.The middle part of this frame is locked as usu- al.Two end of the frame is slidable.So the spine longitudinal growth and vertebral transverse growth is ot limited.Resut:In this study,98 cases average pre-operation Cobb’s angle is 66.43°,post-opera- tion Cobb’s angle is 21.21°,the correction rate is 68%.47 cases was followed up for 6 to 36 month,to- tal longitudinal growth is 55.4cm,average longitudinal growth is 1.2cm.Conclusion:Slidable pedicle screw frame is stable and effective when it was used to treat spinal deformity in growth period,Follow -up result showed that longitudinal growth of spine is not limited and no stiff,rotation deformity and crank shaft found.
