【摘要】 目的:比较两种新型机用镍钛旋转根管器械Mtwo与ProTaper在老年患者磨牙根管治疗中应用的临床效果。方法:选取临床上因磨牙牙髓炎或根尖周炎需要进行根管治疗的年龄在65～86岁间老年患者56例患牙62颗,共217个根管,随机分成两组,采用crown- down根管预备技术,一组应用机用镍钛旋转器械Mtwo进行根管预备,治疗31颗患牙102个根管:另一组应用机用镍钛旋转器械ProTaper进行根管预备,治疗31颗患牙115个根管。两组均采用冷牙胶侧方加压技术根管充填:记录每个根管预备所需要的时间及器械折断的数目,根据治疗前、中、后的X线片评价预备及根充效果。结果:Mtwo组没有发生器械分离,平均每个根管预备时问为96秒;ProTaper组有4支器械(3支F1和1支F2)发生根管内器械折断,4个根管均在显微超声技术下取出根管内折断器械并完成根管治疗,平均每个根管预备时间为107秒;X线片评价两组患者所有的根管预备锥度及流畅度好,根管形态及工作长度无改变。结论:在老年患者磨牙根管治疗中,使用机用镍钛旋转器械Mtwo与ProTaper进行根管预备,均可获得较好的临床效果,但ProTaper更容易发生器械分离。更多还原

【Abstract】 Objective:To evaluate the clinical effects of root canal therapy of geriatric molars with two new Ni- Ti rotary instruments Mtwo and ProTaper.Method:62 Teeth (217 root canals) with pulpitis or periapicitis of 56 65-86 aged patients were included in this study,they were divided into 2 groups.Group I included 102 root canals was prepared by Mtwo NiTi rotary instruments with crown-down technique,the other was ProTaper.All the prepared root canals were obturated with lateral compaction technique with cold gutta-perch.The time of the preparation of each root canal was recorded and analysed.The effect of preparation and obturation were evaluated with radiographs.Results:The NiTi instrument remained better centered of the canal and could achieve continu- ously tapered root canal shape.No instrument was separated in the group 1,the mean time of preparation of each canal was 96 second;but 4 instruments were separated in the group 2;3 were F1 and 1 was F2,all the separated instruments were successfully managed with the micro-ultrasonic technique,the mean time was 107 second. Conclusions:Good clinical effect is found in the geriatric molar root canal therapy With Mtwo or ProTaper rotary NiTi instruments,but ProTaper is easier to be separated during the preparation.更多还原
