【摘要】 目的探索特殊部位囊性病变微创治疗的方法和价值。方法在CT导向下,确定囊性病变的穿刺点、穿刺角度和深度,在局麻下进行穿刺,抽出囊液后,根据囊液的性质注入不同的药物进行治疗。结果共治疗257例特殊部位的囊性病变,包括:真性囊肿(含上皮细胞): 肝囊肿(多囊肝)22例,肾囊肿(多囊肾)57例、甲状腺囊肿6例,肠系膜囊肿4例,腹膜后淋巴管瘤5例,卵巢囊肿13例,肺囊肿4例,支气管囊肿3例;假性囊肿(炎性假瘤):胰腺假性囊肿9例,胸腔包裹性积液14例;血肿:颅内血肿24例,颅内硬膜外血肿5例．硬膜下血肿2例,腹腔积血4例,胸腔积血3例:脓肿:肝脓肿13例．脾脓肿6例．肾周脓肿5例, 脓胸8例,腹腔手术后脓肿6例,肝包虫囊肿4例;囊性肿瘤:颅内囊性胶质瘤12例,颈部神经鞘瘤囊变5例,囊性胸腺瘤3例,腹腔囊腺瘤5例,骨囊肿2例。肝海绵状血管瘤13例。效果确切有效。结论用CT导向介入治疗全身各处特殊部位的囊性病变,是一种创伤小、操作简便、安全可靠、适应证广的先进的治疗手段,值得推广使用。更多还原

【Abstract】 Objective To assess method and value of interventional treatment for cystic lesions in special sites is performed. Method The puncturing point, angle and depth of cystic lesions were located on CT. The liquid in cyst was drawn out after local anesthesia, and then different medicine was injected in it according to the nature of the liquid. Result A total of 257 cystic lesions in special sites were performed. Among them, 114 real cysts (with epithelial cells) including 22 hepatic cysts (polycystic liver), 57 renal cysts(polycystic kidney), 6 thyroid cysts, 4 mesenteric cysts, 5 retroperitoneal lymphangiomas, 13 ovarian cysts, 4 pulmonary cysts, 3 bronchial cysts; 23 pseudo-cysts (inflammatory pseudotumor) including 9 pancreatic pseudo-cysts and 14 encapsulated pleural effusion; 38 hematomas including 24 intracranial hematomas, 5 intracranial epidural hematomas, 2 subdural hematomas, 4 hemoperitoneums, 3 hemathorax; 42 abscesses including 13 hepatic abscesses, 6 splenic abscesses, 5 perinephric abscesses, 8 empyemas, 6 peritoneum postoperative abscesses, 4 hydatid cysts of liver; 40 cystic tumors including 12 intracranial cystic gliomas, 5 cystic change of cervical neurilemmoma, 3 cystic thymomas, 5 peritoneal cystadenomas, 2 bone cysts, 13 cavernous hemangiomas of the liver. Efficacy of this therapy has been proved to be reliable. Conclusion The CT-guided interventional treatment for cystic lesions in special sites is an advanced therapy that’ s micro-traumatic, easy-performed, safe and widely adapted to different cystic sites.
