国内首次从大肠埃希菌发现blaCTX-M-55型超广谱β内酰胺酶基因

【摘要】 目的了解我院产超广谱β内酰胺酶大肠埃希菌耐药基因表达状况。方法34株大肠埃希菌分离自我院感染病人,PCR法检测16种β内酰胺酶基因,包括blaTEM、blaSHV、blaCTX-M-1群、blaCTX-M-2群、blaCTX-M-9群、blaOXA-1群、blaOXA-2群、blaOXA-10群、blaPER、blaGES、blaVEB、blaCARB、blaDHA、blaACT-1、blaMOX、blaLAT/CMY,结果34株大肠埃希菌中有31株携带blaTEM基因,10株携带blaCTX-M-1群基因,4株携带blaOXA-10群基因,阳性率分别为91.2%、29.4%、11.8%。通过DNA测序和BLAST比对分析发现blaCTX-M-55亚型。结论大肠埃希菌携带多种超广谱β内酰胺酶耐药基因,限制了β内酰胺类抗生素在临床的应用。本研究首次报道了blaCTX-M-55亚型超广谱β内酰胺酶耐药基因。
【Abstract】 OBJECTIVE To investigate resistant phenotype of Extended-spectrum beta-lactamases producing Escherichia coli in our hospital.METHODS Thirty-four E.coli isolates were collected from patient.Polymerase chain reaction was used to detected sixteen resistant phenotype of Extended-spectrum beta-lactamases including blaTEM,blaSHV, blaCTX-M-1 group,blaCTX-M-2 group,blaCTX-M-9 group,blaOXA-1group, blaOXA-2group,blaOXA-10 group,blaPER,blaGES,blaVEB,blaCARB,blaPHA, blaACT-1,blaMOX,bla(AT/CMY).RESULTS Of the 34 isolates,31 carried blaTEM,10 carried blaCTX-M-1 group,4 carried blaOXA-1group.The positive rates were 91.2%,29.4%, 11.8%respectively.In blaCTX-M-1 group beta-lactamases producing E coli,CTX-M-55 subtype was found with DNA sequence and blast sequence alignment.CONCLUSIONS Clinical E coli isolates carried several resistant phenotype of Extended-spectrum beta-lactamases,which caused serious resistance to beta-lactam antibiotic.This study is the first published report of CTX-M-55 subtype Extended-spectrum beta-lactarnases in china.
