【摘要】 目的:探讨喙锁螺钉技术的改良及微创化应用初步总结。方法:43例单侧新鲜肩锁关节脱位, 均为急性创伤所致,其中男23例,女20例,平均32.6岁,采用2cm+6mm 双切口、微创喙锁螺钉手术治疗: 2cm 切口用于直视复位、修复肩锁韧带;6mm 切口用于实施充分微创化的喙锁螺钉技术,准确定位,在锁骨及喙突根部钻孔(使用钻套保护喙锁间软组织;钻孔深度控制器避免喙突下血管神经损伤),扩大锁骨孔并用螺钉沉头器处理;避免对喙突孔攻丝以防止喙锁螺钉失效;测量器选择合适的螺钉,以拧紧后喙锁关节间距正常、螺钉尖端刚好穿透喙突双层皮质为准,纠正肩锁关节脱位并维持固定直至愈合,同时修复肩锁韧带及关节囊结果42例获得随访,随访时间5月至3.9年,平均1.9年,疗效按 Karlsson 分类进行评定:A 级40 例、B 级2例结论喙锁螺钉技术经改良、对手术入路微创化处理后,具有技术稳定性好、疗效可靠、并发症少等优势,并且创伤小、切口短、美观、操作简便、经济实惠,值得推广。更多还原

【Abstract】 Objective:To research the micro-traumatic technique of coraco-clavicular screw to fix acro- mioclavieular dislocation.Methods:Forty-three patients with fresh acromioclavicular dislocation were operationed by micro-traumatic technique:the incisions length is 2cm And 6mm,Then the clinical data were analyzed and summarized.Results:The patientas follow-up has progressed during 5 months to 3.9. years(the average number is 1.9 years),forty-two patients have been follow-up visited.They were divided into two groups by the classification of Karlsson:forty by grade A,two by grade B.Conclusion:The micro-trau- matic technique of coraco-clavicular screw to fix acromioclavicular dislocation have many advantages,such as the short incisions、small trauma、little complications、simple operation、low cost and satisfied curative effect,and so on.更多还原
