乙肝病毒转录体在母婴传播中的应用

【摘要】 目的探讨乙型肝炎病毒(HBV)转录体的检测在母婴传播中的意义。方法自母亲及其婴儿血清中提取核酸,经 PCR 及 RT-PCR 分别扩增 HBV DNA 和 RNA,琼脂糖凝胶电泳显示产物,Southern-blotting 验证反应的特异性,取代表性产物克隆、测序。结果 HBeAg(+)母亲的 XDNA(75%)和 fRNA 的表达(65%)显著(P〈0.05〉)高于 HBeAg(-)的母亲;所有表达 fRNA 的母亲也同时表达 trRNA.在20例 XDNA(-)fRNA(-)的母亲中,仍有8个可检测到 trRNA。婴儿 XDNA(4.55%)和 fRNA(6.81%)的表达较其母亲显著降低;占优势表达的是 trRNA(52.27%)。且与其母亲 trRNA 的表达状态密切相关。母亲为 trRNA(+)的婴儿 trRNA 的表达显著高于母亲为 trRNA(-)者。15例 HBsAb(+)的婴儿均未检出 XDNA 和 fRNA,但11例仍可检测到 trRNA。结论与 HBV DNA 及其表达产物相比,trRNA 在 HBV 母婴垂直传播过程中有可能是一个出现更早期的可检测指标,它有助于更精确的确定新生儿 HBV 感染的状态。
【Abstract】 AIM To investigate the charicteristics and clinical significance of HBV transcripts(the full length and truncated RNA molu- cules)in thansmitting from mother to infant.METHODS The serum viral nucleic acids extracted from pairs of mother and infant were an- alyzed using PCR and RT-PCR targeted to the HBV X pintein-encoding region.Reaction specificity was confirmed by Southern blotting. Representative amplification products were cloned and sequenced.RESULTS The expression of XDNA(75%)and fRNA(65%)in HBeAg(+)mothers was significantly higber than that in HBeAg(-)mothers(P〈0.05〉).All the mothers expressing fRNA expressed XD- NA and trRNA at the same time.8 mothers out of 20 were trRNA(+),who were both XDNA(-)and fRNA(-).The detection rate of XDNA and fRNA in infants were significantly lower than that in their mothers,whereas the detection of trRNA was high(52.27%),which was expressed superiorly in infants serum.Infants’ trRNA expression was associated closely with their mothers’ that infants from trRNA (+)mothers expressed trRNA higher than those whose mother were trRNA(-).In 15 HBsAb(+)infants who were both XDNA(-)and fRNA(-),11 still can be detected trRNA.CONCLUSION Compared with HBV DNA and HBV gene expressing products,trRNA might be a earlier detectable serum maker for identifying HBV infection profile accurately in infnats,which was associated closely with their moth- ers’ expression of trRNA.
