负荷-再分布201T1心肌灌注显像结果的可比性研究

【摘要】 目的探讨常规负荷-再分布201T1心肌灌注显像结果的可比性。方法随机抽取201T1心肌灌注显像患者30例。8min内采集负荷数据,220min左右采集再分布数据,两次数据采集的条件和重建参数完全一致。分别计算:①负荷、再分布总计数的平均值及二者差值的平均值、再分布总计数下降的平均百分比;②相同角度、相同层面和相同大小感兴趣区(ROIS)内心肌、肝脏、肺部和甲状腺在负荷、再分布时的平均计数及ROIS两次总计数差异的平均值,以及心肌、肝脏、肺部和甲状腺ROIS再分布计数下降的平均百分比;③负荷、再分布每帧平均计数率及二者差值的平均值、再分布每帧平均计数率下降的平均百分比;④要达到负荷时总计数水平,再分布数据采集需要延长的时间。所有数据采用SPSS13.0统计软件分析,相关均数行配对t检验。结果常规显像方法再分布总计数平均下降百分之20.76±5.6,差异具有显著性(P<0.05);与负荷时比较,心肌、肝脏、肺部和甲状腺ROIS再分布计数分别平均下降百分之34.83±7.4、43.14±12.00、29.24±6.24和11.44±7.80,差异均具有显著性(P<0.05);再分布每帧平均计数率平均降低约百分之29.60±4.95,差异具有显著性(P<0.05);再分布每帧数据采集实际所需时间应在负荷基础上平均延长约百分之42.61±9.59,或17.04s±3.80s。结论常规负荷-再分布201T1心肌灌注显像结果可能由于计数水平的显著差异而缺乏可比性,但可以通过延长再分布数据采集时间得到改善。

【Abstract】 Objective This paper was to probole into the comparabiblity for the regular stress-redistribution myocardial perfusion imaging with 201T1.Methods Emmision data were acquired within 8rain after stress and 3~4h later for redistribution images using the same imaging parameters,respectivily,on 30 random cases with ADAC VERTEX V60 PLUS SPECT.The same reconstrucion parameters were used for both stress and redistribution image reconstructing.Total counts of stress and redistribution imaging of each case were recorded for calculating the average values of stress counts,redistribution counts,counts difference between stress and redistribution、counts reduction ratio of redistribution imaging compared with that of stress imaging.Then the similar average values were also calculated in the areas of myocardio、liver、lungs and thyroid regions of interet(ROIs),which included in the field of view of the detectors,from the original stress and redistribution imaging data respectivily.And the average count rate between stress and redistribution imaging were compared.Fanally,the data acquisition time per fram should be increased for the redistribution imaging,based on the stress imaging,were calculated.The SPSS13.0 software and paired-sampler test were used for the statistics analysis.Results The average reduction ratio of the total counts、ROIs counts of myocardio、liver、lungs and thyroid and count rate of the redistribution imaging,3~4h later,were 20.76±5.60、34.83±7.40、43.14±12.00、29.24±6.24、11.44±7.80 and 29.60±4.95 per cent(P<0.05),respectivily.The average increasing ratio of the acquiring time each fram for the redistribution imaging was 42.61per cent,or 17.04s±3.80s per fram should be increased compired with the stress imaging.Concalutions The results of the regular stress-redistribution myocardial perfusion imaging with 201T1 malght beyond comparison,but it would be improved by lasting the acquisition time for the redistribusion imaging.
