FDG-PET/CT显像在意外发现第二原发恶性肿瘤中的价值

【摘要】 目的:探讨18F-脱氧葡萄糖(FDG)全身 PET/CT 显像在对肿瘤进行诊断和分期时意外发现另一原发恶性肿瘤的价值方法:对2005年3月至2007年4月的2229名患者进行18F-FDG PET/CT 全身显像,以达到对肿瘤的诊断和分期结果:有20例在全身 PET/CT 显像中意外发现第二原发恶性肿瘤,其中16例被病理证实。恶性肿瘤的位置包括肺(25%),胃肠道(15%),头颈部(25%),乳腺(10%),妇产科(5%),生殖泌尿系统(5%),食管(5%),胰腺(5%) 和淋巴结(5%)。组织学证实为第二原发恶性肿瘤的包括胃肠道肿瘤(6例)、肺癌(2例)、肝癌(2例),恶性淋巴瘤 (2例),乳腺癌和食管癌(2例),鼻烟癌(1例),甲壮腺癌(1例),肠系膜恶性间皮瘤(1例),胆囊癌(1例),肾癌(1 例)和卵巢癌(1例)。结论:18F-FDG PET/CT 全身显像对意外发现第二原发恶性肿瘤有重要意义。

【Abstract】 Purpose:To evaluate the value of combined whole-body positron emission tomography(PET)and computed tomography(CT)using fluorine-18 fluorodeoxyglucose(FDG)in unexpected detecting an additional primary cancer at the lime of the tumor diagnosis and initial staging.Methods:Reports from whole-body(18)F-FDG PET/CT scans from March 2005 to April 2007 were reviewed,and 2229 patients who had been scanned for the diagnosis and initial staging of tumor were included in this study.Results:FDG PET/CT found 20 lesions indicative of a second primary cancer in 2229 patients (8.79%),of which 16 lesions were proved to be a second primary cancer.The sites of known or suspected primary tumors included lung(25%),gastrointestinal tract(15%),head or neck(25%),breast(10%),gynecologic organs(5%),genitouri- nary organs(5%),esophagus(5%),pancreas(5%)and lymphoma(5%).The histological proved second primary tumors included gastrointestinal tract(6 lesions),lung(2 lesions),liver(2 lesions),lymphoma(2 lesions),breast and esophagus(2 lesions),nasopharyngeal neoplasms(1 lesion),thyroid(1 lesions),malignant mesothelioma in mesentery(1 lesions),gall- bladder(1 lesion),kidney(1 lesion),ovary(1 lesion).Conclusion:FDG PET/CT at the time of diagnosis and initial staging is useful for screening a second primary cancer with a high sensitivity.
