青春期体格因素对骨量指标的混杂影响

【摘要】 目的探讨青春期儿童骨量评价中体格生长的混杂作用,并提出可行的校正方法。方法使用单光子骨矿分析仪测定161名8-20岁儿童、青少年非优势便4桡骨远端1/3处骨矿含量和骨宽,采用相关分析及多元线性回归分析体格生长指标对骨矿含量的影响。结果男、女性年龄、骨宽、身高和体重均是骨矿含量的显著性影响因素;除12-14岁组外,采用骨宽进行校正得到的骨密度值可较好消除骨骼体积的混杂作用。结论青春期儿童骨量评价中需考虑年龄、骨宽、身高和体重等因素影响;骨密度值虽可在一定程度消除骨宽的影响,但在儿童快速生长发育时期仍应慎用。采用多元线性回归模型可以消除骨宽、身高和体重的混杂作用。
【Abstract】 Objective This study is to discuss the potential size-related artifacts in the bone mass determination during Childhood and puberty. Methods The bone mineral content (BMC) and bone width (BW) of the 161 healthy children and adolescents were assessed by single-photon absorptiometer .The possibility of size-related artifacts is determined using correlation analysis and multivariate linear regression. Results Positive correlation between bone mineral content and age , bone width , height , weight were observed in both males and females. Size adjustment by using such predefined index as BMD (BMC/BW) can correct BMC fully for bone size except those during 12-14 years old. Conclusion Age , bone width , height and weight are important factors that correlate to BMC. All these should be matched correctly during assessment of BMC in epidemiological studies. Although BMD can correct BMC fully for bone size, it failed to adjust the bone size in those of 12-14yr adequately. The recommended way to avoid this is to use multivariate linear regression models that include such independences as age , bone width , height , weight.
