阿尔茨海默病患者甲状腺激素与皮质醇测定的临床意义及其相关性

【摘要】 目的探讨阿尔茨海默病(AD)患者血清甲状腺激素与皮质醇(CORT)水平的变化以及它们的相关性。方法采用简易精神状态量表(MMSE)对77例AD患者和100例同龄健康人进行评分,对77例AD患者和100例健康对照组进行甲状腺激素(TT3、FT3、TT4、FT4、TSH、TU)和皮质醇(CORT)的测定,并进行组间对比研究和相关性研究。结果 AD患者TT3、FT3水平明显低于健康对照组,两者差异有极显著性(P<0.001.,P<0.001):TU、皮质醇水平明显高于健康对照组,两者差异有极显著性(P<0.001,P<0.01)。AD患者TT4、FT4水平低于健康对照组,但差异无显著性(P>0.0S,P>o.05);TSH水平高于健康对照组,但差异无显著性(P>0.05)。AD患者血清皮质醇水平与MMSE得分呈显著负相关,与甲状腺激素水平无显著相关性;AD患者TT3水平与FT3、TT4水平呈显著正相关,TT3水平与TU水平呈显著负相关,FT3水平与TT4水平呈显著正相关,FT3水平与TU水平呈显著负相关,TT4水平与FT4水平呈显著正相关。结论甲状腺功能异常可能与AD的发病有关,血清皮质醇水平的增高可能与AD病程的进展有关。

【Abstract】 Objective:To discusses the clinical significance of determination of serum thyroid hormones and Cortisol(CORT) in patients with Alzheimer disease(AD) and their Correlations.Methods:To measure serum concentration of thyroid hormones(TT3、FT3、TT4、FT4、TSH、TU) and Cortisol(CORT) in 77 patients with AD and 100 normal controls respectively.Aftet grouping by degree of dementia with Clinical Dementia Rating Scale.The results were compared between each two groups,and carried on the relevant research.The mental state was scored using Mini-mental state examination(MMSE).Result:The serum TT3 and FT3 level in AD was significantly lower than that of normal controls(P<0.001,P<0.001).The serum TU and Cortisol level was significantly higher than that of normal controls(P<0.001,P<0.01).The serum TT4 and FT4 level in AD was lower than that of normal controls without statistical significance(P>0.05,P>0.05). The serum TSH was higher than that of normal controls without statistical significance(P>0.05). Significant negative correlation existed between serum Cortisol level and MMSE scores.No correlation existed between serum Cortisol level and Serum Thyroid Hormones level.Significant positive correlation existed between serum FT3 level and serum TT4 level,While significant negative correlation existed between serum FT3 level and serum TU level.And significant positive correlation existed between serum TT4 level and serum FT4 level.Conclusions:abnormal serum thyroid hormones level may be related with AD.And the serum Cortisol level is possibly related with the AD’s progress.
