【摘要】 目的探讨长期接受高效联合抗反转录病毒治疗（Highly active antiretroviraltherapy,HAART）后的AIDS患者的治疗反应、免疫重建和副作用。方法对39例艾滋病患者服用国产抗病毒药物治疗36个月过程中的临床资料(包括淋巴细胞亚群、CD₄₅RA细胞表型百分数、病毒载量、临床症状体征、卡洛夫斯基积分、体重等)进行回顾性分析。结果 39例AIDS患者的CD₄⁺T淋巴细胞计数在疗后6、12、24、30、36个月较疗前升高,有显著的统计学差异（P<0.01）;CD₄⁺T淋巴细胞计数在疗后24、30、36个月较疗后12个月升高,有显著的统计学差异（P<0.01）;CD₄⁺T淋巴细胞计数疗后第33个月较第24个月略有下降,有统计学差异（P<0.05）,CD₄⁺T淋巴细胞计数疗后27、30、36个月较24个月时略有下降,无统计学意义。乏力、纳呆、呕吐、气短、自汗、盗汗、恶心、头痛、腰痛、皮肤瘙痒、皮疹出现的频率和或严重程度降低,其中乏力、纳呆、气短得以显著改善;疗后24个月至36个月连续12个月的病毒载量基本保持稳定;AIDS患者CD₄₅RA表型百分比在疗后27、30、33、36个月时较24个月时无统计学差异（P>0.05）;患者体重基本保持稳定。结论长期应用HAART可有效控制HIV复制,升高CD₄⁺T淋巴细胞计数,促进免疫重建,显著改善了艾滋病患者的生活质量,但对免疫系统功能的长期恢复作用不明显。更多还原

【Abstract】 Objective The patients therapeutic effect,immunological reconstruction and adverse reaction were investigated through long-term administration of highly active antiretroviral therapy（HAART）.Methods Clinical information（including lymphocyte subgroup、CD45RA phenotype percent、HIV viral lord、signs and symptoms、Karnovsky Score、weight）of 39 cases of AIDS patients who were given long-term administration of HAART were collected and analyzed.Results CD4 count of the patient increased in month6,12,24,30,36 compared with pre treatment.There is obvious statistic difference（P<0.01）.CD4 count of the patient increased in month24,30,36 compared with in monthl2.There is obvious statistic difference（P<0.01）.CD4 count of the patient decreased in month 33 compared with in month24.There is statistic difference（P<0.05） CD4 count of the patient decreased in month 27,30,36 compared with in month24.There is no statistic difference（P>0.05）.Clinical signs and symptoms are alleviated such as fatigue,anorexia,vomiting,short of breath,spontaneous sweating,night-sweat,nausea, headache,lumbodynia,itch of skin and rash,especially fatigue,anorexia,short of breath.There is remarkable statistic difference between pre and post treatment.Viral loads were stable during 24 month to 36 month.There were no differences in CD45RA percent.Weight of the patients maintains stable.Conclusion Through long-term administration of highly active antiretroviral therapy,HIV reproduction can be inhibited substantially,CD₄⁺T cells are improved before treatment,immunological reconstruction are promoted.Signs and symptoms are alleviated and quality of life of the patients is improved.However the long term recovery for function of immune system is not very obvious.更多还原
