【摘要】 目的:研究正常生育力(无避孕同居生活12个月内使配偶获得妊娠)男性精液质量水平,并分析到达妊娠时间((time to pregnant,TTP)与精液参数相关性。方法:招募年龄在20-50岁之间,妊娠到达时间12个月内的男性,填写问卷、体检和精液分析,计算精液参数正常参考值范围,探讨TTP和其他生物学特征对精液质量的影响。结果:1152位年龄范围18-48岁的男性接受精液检查,精液体积(m1)、精子浓度(10~6/ml)、总精子数(10~6)、活动率PR+NP(%)、前向运动精子PR(%)、正常形态精子百分数(%)、精子存活率(%)、前向运动精子总数(10~6)第5个百分位数值分别为1.5、17、39、41、33、5、53和17。年龄≥35岁组对象前向运动精子总数较少(p<0.01);高体质量指数(BMI>25.0)组对象正常形态精子百分率较低(p<0.01),低体质量指数(BMI<18.5)组精子浓度、总精子数、正常形态精子百分率和前向运动精子总数较低(p<0.05);吸烟降低精液体积(p<0.01);饮酒超过10g/周减少精液体积、总精子数、正常形态精子百分率和前向运动精子总数(p<0.05);TTP与对象精子浓度、总精子数和前向运动精子总数呈负相关(p<0.01);对象禁欲时间5-7天组的精子浓度、总精子数和前向运动精子总数高于其他组(p<0.01)。结论:正常生育力男性精液参数第5个百分位数值与世界卫生组织的正常参考值范围基本相同;TTP数值与精液质量呈负相关。年龄、体质量指数和生活方式会对部分精液参数产生影响。更多还原

【Abstract】 Objective:To study the semen quality of proven fertile men with time to pregnancy (TTP) of 12 months or less,and analyze the correlation of sperm parameters with TTP.Methods: 1152 men were recruited and semen analysis was done according to WHO manual 2010.Normal reference ranges of semen parameters were calculated.Effect of TTP and other biological characteristics on sperm quality were investigated.Results:The fifth centiles of semen parametres were generated:semen volume,1.5 ml;sperm concentration 17 million per ml;total sperm number, 39 million per ejaculate;total motility,41%;progressive motility,33%;normal forms,5.0%; vitality,53%live;total number of progressive sperm,17 million per ejaculate.Age,BMI, abstinence time,smoking and alcohol abuse may decrease semen quality.There was negative correlation between TTP with sperm concentration,total sperm number and the total number of progressive spermatozoa.Conclusion:The fifth centiles were close to the WHO normal reference ranges for semen parameters in fertile men.TTP was in negative correlation with semen quality. Age,BMI and lifestyle would affect some of the semen parameters.更多还原
