【摘要】 目的:为了解McKissock法巨乳缩小术与双环法巨乳缩小术及垂直短疤技术巨乳缩小术的近期及远期差异点。作者对上述三种方法各选择了一组病例进行比较。方法:50例巨乳患者(99只巨乳),分别采用McKissock法巨乳缩小术17例(34只乳房);双环法巨乳缩小术17例(33只乳房);垂直短疤技术巨乳缩小术16例(32只乳房)。结果:比较近期、远期疗效的优、缺点。外形、功能、切口瘢痕、感觉、手术适应症、手术设计、乳腺切除范围、手术禁忌症等(近期远期外形变化,瘢痕种族差异,哺乳功能保留,乳头、乳晕感觉功能保留,乳血液供应、神经支配等)。结论:McKissock法巨乳缩小术、双环法巨乳缩小术及垂直短疤技术巨乳缩小术各有优缺点。McKissock法巨乳缩小术近期外形良好,但乳下极倒T状切口瘢痕及远期乳下极外凸畸形为其不足;就亚裔人种而言,双环法巨乳缩小术具有切口瘢痕隐蔽,避免了乳下极倒T状或竖状切口瘢痕,保留了哺乳功能,但其缺点为乳外形略显扁平,有个别切口瘢痕明显;垂直短疤技术巨乳缩小术乳房呈维体状,外形优美、挺拔,近期乳下极竖状切口融合处呈皱缩状,这一点术前必须向患者说明。远期乳下极仍遗留竖状切口瘢痕,其远期疗效有待进一步观察。更多还原

【Abstract】 Objective:To investigate short-dated and longdated effects of McKissock,double-circle and vertical short scar reduction mammaplasty techniques. Methods:50 macromastia patients(99 breasts) were involved in the study.17 patients(34 breasts) were undergone McKissock reduction mammaplasty,17 patients (33 breasts) were undergone double -circle reduction mammaplasty and 16 patients(32 breasts) were undergone vertical short scar reduction mammaplasty. Results:Advantages and disadvantages of short-dated and long-dated effects were compared from following aspects: shape,function,scar,feeling,operation indications, operation designing,range of mastectomy,operation contradictions et al(short -dated and long -dated shape changes,race differences of scar formation,preservation of lactation,preservation of nipple and areola feelings,blood supply of breasts,innervation et al). Conclusion:McKissock,double -circle and vertical short scar reduction mammaplasty techniques,they each has both advantages and disadvantages.McKissock technique provides nice short-dated shapes,while inverted T-shaped scar and long -dated convex deformity in inframammary region are its disadvantages.To Asian races,double-circle technique has advantages of possessing hidden scar, avoiding inverted T -shaped or vertical scars in inframammary region and maintaining lactation function, while its disadvantages are relatively flat-shaped breasts and obvious scars in several cases.After vertical short scar reduction mammaplasty,breasts are pyramidal,graceful and straight.There will be short-dated vertical shrinkage-like seams in inframammary region,which should be explicated to patients before surgery.Long-dated vertical scars will be existed in inframammary region,and its long-dated effects need to be further studied.更多还原
