【摘要】 目的:探讨输尿管镜下钬激光治疗并发息肉的输尿管上段阴性结石的疗效。方法:2008年1～12月采用输尿管镜下钬激光(美国Coherent PowerSuite 20W激光系统,光纤500μm)术治疗合并息肉的输尿管上段阴性结石60例。经尿道输尿管镜直视下到达结石部位,采用钬激光腔内碎石治疗,同时消融息肉。结果:手术时间15～70 min,平均21 min。60例息肉均一次治疗成功;除4例上段结石被冲入肾盂外,其余56例均单次原位碎石成功。术中未发生输尿管黏膜撕脱、输尿管穿孔、输尿管黏膜隧道等严重并发症。术后仅有轻微血尿,无低热、肾区疼痛等不适。术后随访3月～12个月均未发现结石残留及输尿管狭窄。结论:输尿管镜下钬激光治疗合并息肉的输尿管上段阴性结石安全,疗效满意。更多还原

【Abstract】 Objective:Discuss about the effect,safety and related skills of holmium laser lithotripsy treatment of upper urinary tract radiotransparent calculi accompanied with polyps via ureteroscope1. Method:60 cases of holmium laser(Coherent PowerSuite 20W,500μm)lithotripsy treatment of upper urinary tract radiotransparent calculi accompanied with polyps via ureteroscope1from January 2008 to December 2008.the laser lithotrite was introduced to the stones,and holmium laser intraluminal stone fragmentation was carried out,which were melted by holmium laser simultaneously. Result:Polyps was removed successfully by just once treatment in all 60 cases,only in 4 upper calculi cases the calculi was flushed into renal pelvis,and in all other 56 cases,the calcus is smashed successfully in origin position and just through once treatment,The operation time varies f tom 17 minutes to 70 minutes,and the average time is 21 minutes. No ureteral mucosa avulsion,perforation,false passage and other severe complications occurred.Only some mild haematuria occurred,no low fever,pain in renal region and other discomfort s are found.The postoperative follow-up of 3 months to 12 months revealed Acalculous residues and no ureteral stricture. Conclusion:The holmium laser lithotripsy t reatment of upper urinary tract radiotransparent accompanied with polyps via ureteroscope is a kind of effective and safe method.更多还原
