脑卒中发病时间生物学研究
Chronobiological study of stroke attack

【摘要】 目的探讨急性脑卒中发病的时间规律,为时间预防和治疗脑卒中提供理论依据。方法收集烟台毓璜顶医院2005-2006年急性脑梗死病例1175例,男722例(65.1±12.3岁),女453例(65.8±11.9岁)。急性脑出血病例330例,男182(59.8±11.8岁),女148(65.3±12.9岁)。详细记录患者的发病时刻(时分)及日期。应用SPSS10.0软件进行数据分析。结果1)有明确发病时刻记录的脑梗死664例、脑出血218例进入昼夜时间分析;脑梗死、脑出血患者男性发病明显多于女性,脑出血男性患者的平均年龄明显小于女性;2)脑梗死患者发病时间呈现明显的昼夜节律性变化(P<0.01),发病高峰在上午(6:01-12:00,42.2%),峰值在6:01-8: 00(15.4%),夜间发病较少,谷底在0:01-2:00(4.2%)。未见发病的明显周节律和季节节律性变化(P>0.05); 3)脑出血患者发病时间呈现明显的昼夜与季节性变化(P<0.01),昼夜发病高峰在上午(6:01-12:00,35.9%),峰值在6:01-8:00(14.3%),夜间发病较低,谷底在2:01-4:00(5.2%),其中合并高血压者发病呈现双峰,分别在6:00-10:00(28.6%)和16:00-20:00(22.1%);未见脑出血发病呈现周节律,但呈现明显的季节节律,冬季12月明显高于夏季7月(P<0.01)。结论脑卒中发病时间存在明显的节律性。脑梗死与脑出血发病时间均存在明显的昼夜节律性,脑出血发病存在季节性变化。
【Abstract】 Objective To explore the rhythmicity in the onset of stroke,the result may provide evidence for the prevention and treatment of the patients with stroke.Methods 1175 hospitalized acute cerebral infarction patients and 330 hospitalized crebral hemorrhage patients from Jan.1st,2005 to Dec.31th,2006 in Yuhuangding Hospital.Among cerebral infarction patients,722 men,aged 65.05±12.31yrs,and 453 women,aged 65.83±11.86 yrs.Among crebral hemorrhage patients,182men,aged59.77±11.799 yrs,and 148women,aged 65.33±12.88 yrs.We register the exact time of the onset-the hour,the week,and the month.We deal with the data with statistical method,and to check whether it has significant difference.Results The number of male are more than the number of female in all subject(CI、CH),The average age ofmaleare less than the female in CH patients.In the analysis ofall CI(Cerebral infarction) patients,6:01-8:00(15.4%)of one day whichformed a dominant morning peak and much more than other time(p<0.01),and 42.2%of episodes occurred between 6:01-12:00(P<0.001).Aweekly rhythm and seasonal variation of CI could not be demonstrated.In the analysis of all CH(Cerebral hemorrhage)patients, 35.9%of episodes occurred between 6:01-12:00,and of 6:01-8:00(14.3%)show a peak(p<0.01).A weekly rhythm ofCH could not be demonstrated.The seasonal variation of CH onset in totle subject tend to show one peak in winter(P<0.01).Conclusion The onset of stroke exhibit a circadian rhvthm.It showed a single peak in the period from 6:01-8:00 in CI and CH.The seasonal variation of CH onset in totle subject tend to show one peak in winter.
