【摘要】 目的究速度依赖性平板训练改善小腿截肢患者承重和步行能力的作用。方法60例小腿截肢患者(入选标准:单侧小腿截肢患者,34岁-54岁,无运动限制性疾病,能应用假肢步行16米,VAS<5/10)采用随机对照研究和组内重复测量的方法,分为常规治疗组(A组)、常规治疗加均速度组(B组)和常规治疗加速度依赖性治疗组(C组),根据10MWT确立运动平板处方,分别采用常规治疗,均速度和速度依赖性两种平板训练方案进行承重和步行训练与观察;分别在训练前、训练2w、4w、6w和8w后对10MWT、最长步行距离和静态承重进行测评和分析。应用SPPSS10.0进行统计三组之间用one-way ANOVA,组内应用repeated-measures ANOVA对数据进行分析。结果过8W康复治疗后,所有患者功能均改善明显,治疗前后相比差异具有显著性意义(p<0.01);治疗后,应用平板训练的患者负重和步行能力均优于常规治疗组(p<0.01),且常规治疗加速度依赖性治疗组优于常规治疗加均速度组,差异具有显著性意义(p<0.01)。结论用速度依赖性平板训练对小腿截肢患者功能改善最佳,均速度平板训练次之,常规治疗效果最慢。更多还原

【Abstract】 Object:To study the effects of speed-dependent treadmill training on weight bearing and walking ability in patients with below knee amputation.Method:60 patients who suffered below knee amputation in single leg were randomly devided into three groups:traditional treatment group(group A), traditional treatment and treadmill training with same speed(group B),traditional treatment and speed-dependent treadmill training(group C);During the treatment,measure the 10 meter walking test (10MWT),Maximum walking distance and static weight bearing in the end of 2 weeks,4 weeks,6 weeks and 8 weeks.Use the software of SPSS 10.0 to analysis the data with one-way ANOVA for inter-group and repeated-measures ANOVA in intra-group.Result:After 8 weeks,the weight bearing ability and walking ability were significantly improved in all three groups(p<0.01);all of the test are improved in treadmill training compared with traditional treatment group(p<0.01),but the improvement of group C is obvious than group B,and the difference was significant(p<0.01).Conclusion:The treadmill training with speed-dependent has best effects on walking ability and weight bearing for patients with below knee amputation,treadmill training with equal speed was better.更多还原
