【摘要】 目的:探讨微波消融联合TACE治疗原发性肝癌(Primary heptocellular carcinoma PHC)的临床疗效。方法:82例PHC患者随机分为两组,单纯肝动脉栓塞化疗术(TACE)组36例(对照组),微波消融+TACE 46例(治疗组)。观察两组治疗后3个月的临床疗效和AFP变化情况及两组1、2、3年生存率。结果:AFP变化情况,对照组和治疗组由治疗前的479.3±96.2μg/L和,491.2±103.1μg/L降为治疗后的115.3±49.3μg/L和78.1±38.9μg/L。1、2、3年生存率:治疗组为78.3%、54.3%、34.8%;对照组为61.1%、36.1%、19.4%。治疗组未发生烧伤、出血、胆瘘、胃肠道穿孔和肝功能恶化等并发症。结论:微波消融联合TACE是治疗PHC的一种新的安全有效的治疗方法,可以明显延长PHC患者的生存期,并且优于单纯TACE治疗。更多还原

【Abstract】 Objective:To explore the clinical effect of microwave ablation combined with transcatheter arterial chemoembohzation(TACE ) in treatment of primary hepatocellular carcinoma(PHC ).Methods:A total of 82 patients with PHC were randomly divided into two groups:TACE group with 36 patients(control group ),and the other 46 were treated with combination of microwave ablation + TACE(treatment group).After three months’ treatment,the changes in the AFP level and tumor size were analyzed.The 1-,2-,and 3- year survival rates were calculated with Kaplan-Meier method and compared between the two groups.Results:AFP changes,control and treatment groups from pre-treatment 479.3±96.2μg / L and 491.2±103.1μg / L reduced to the after treatment 115.3±49.3μg / L and 78.1±38.9μg,/ L.The 1-,2-, and 3- year survival rates were 78.3%、54.3%、34.8%in treatment group,and 61.1%、36.1%、19.4%in control group,respectively(P<0.01).The treatment group did not burn, biliary fistula,hemorrhage,perforation of the gastrointestinal tract and liver function deterioration and other complications.Conclusion:microwave ablation combined with TACE is a safe and effective in treating PHC,and can achieve better results than TACE alone.It is more effective in decreasing the AFP level and improving survival rates.更多还原
