【摘要】 目的:探讨胆管癌性梗阻的介入治疗的方法和疗效。方法:胆管癌性梗阻的黄疸患者45例,采用经皮肝穿胆管引流术(PTCD)及金属内支架置入术,加局部动脉灌注化疗术或化疗性栓塞术。38例采用经皮肝穿胆管内支架一次成功;7例经皮肝穿经外引流1周后置入金属内支架完成。其中28例置入内支架术后3周在数字减影(DSA)下行局部动脉灌注化疗术或化疗性栓塞术。结果:45例胆管癌性梗阻的黄疸患者,黄疸消退满意,血清总胆红素降至13.6-45.7μmol/L;45例患者随防观察,其中,行金属内支架置入术者平均生存期为10个月,行金属内支架置入术加局部动脉灌注化疗术或化疗性栓塞术者平均生存期最长为15个月,结论:运用介入经皮肝穿胆管引流术(PTCD)加金属内支架置入术,加局部动脉灌注化疗术及化疗性栓塞术是姑息性治疗阻塞性黄疸的有效方法 ,延长了生存期,疗效显著。更多还原
