水冷循环2450MHz微波凝固离体猪肝的实验研究

【摘要】 背景与目的:经皮微波消融术作为一种微创治疗手段,目前正在广泛应用于恶性实体肿瘤的治疗中。本实验拟观察单源及双源水冷循环2450MHz微波天线对肝脏组织的凝固效果,从而更好的指导临床实体肿瘤微波消融治疗。方法:新鲜离体猪肝47个,分别在水冷循环状态下行60w/600s、70w/600s、80w/600s、80w/300s、80w/900s单源微波消融离体猪肝,观察消融区形状,比较不同条件下消融区形态、范围、动态热场分布;双源微波采用60w/600s相距1.5cm和2.0cm行双源微波同时辐射和依次辐射消融离体猪肝,比较两者消融区形态、范围及中间动态热场分布。结果:○1水冷循环微波电极可以明显提高微波凝固输出功率或辐射时间,凝固形态近椭球形或类球型;○2微波辐射时间是影响消融横经和体积的重要指标,微波辐射功率是决定微波场内温度变化得主要因素,远端组织温度上升主要依靠热传导;○3双源微波60w/600s工作方式下,同时进行与依次进行在消融纵径之间比较无统计学差异(P>0.05);双源微波依次进行可以得到更大的消融体积(P<0.05),但双源微波同时进行横/纵径比更接近于1,消融形状更类圆。结论:水冷循环微波可以明显提高微波输出功率和辐射时间,合适的功率时间搭配在微波凝固治疗中十分重要;采用双微波辐射可以得到更适形的凝固范围。

【Abstract】 BANCKGROUND & OBJECTIVE As a minimial invasive treament,percutaneous microwave coagulation therapy(PMCT) is widely used to treat solid malignances clinically.This study was to observe the percutaneous coagulative effects of 2450MHz water circulative-cooling Single and double-electrode microwave ablation in vitro swine liver and to give guidance to our clinical solid malignances microwave ablation. METHODS:47 fresh swine livers in vitro were separately coagulated by the single-electrode with 60w/600s、70w/600s、80w/600s、80w/300s、80w/900s in water circulative condition,investigate and compare the shape、size and dynamic thermal distribution of coagulation lessions under different condition;the double-electrode microwave ablation were processed with 60w/600s,activated either sequentially or simultaneously at the distance of 1.5cm or 2.0cm between two microwave electrodes.RESULTS:○1 the water-cooled electrode can increase the power output and exposure duration,the shape of ablation lessions were all elliptical or circle;o2 the transver and volume were mainly influenced by microwave irradiation time,the power output decided the lever of temperatures in tissue and the distal end tissue temperature depended on heat conduction;○3 the sequential or simultaneous double-electrode microwave ablation with 60w/600s has no statistical significance between two working modes when comparing ablative longitudinal (P>0.05 ),the former one can get lager volume(P<0.05 ),the ratio(transverse/longitudinal) was more to get close to 1 and the ablative shape was more like circle.CONCLUSION:the water-cooled electrode could increase the power output and exposure duration;suitable combination of the power output and the exposure duration is very important in microwave coagulation;double-electrode can get more suitable abalative shape.
