【摘要】 目的:分析天津地区结核分枝杆菌(Mycobacterium tuberculosis MTB)链霉素耐药相关基因rpsL和rrs的突变位点和突变率,为快速诊断链霉素耐药MTB筛选突变检测位点;阐明基因突变类型与北京基因型的关系。方法:PCR扩增116株链霉素耐药MTB与10株链霉素敏感株基因组中rspL和rrs基因,分析突变位点;应用多重PCR鉴定MTB北京家族并分析耐药相关基因突变位点与MTB北京家族的关系。结果:116株链霉素耐药株基因的总突变率为87.1%(101/116)。70株(60.3%)发生rpsL 43 AAG→AGG(K43R)突变,15株(12.9%)发生rpsL 88 AAG→AGG(K88R)突变。17株MTB链霉素耐药株发生rrs突变,包括13(11.2%)株513 A→C突变占,2(1.72%)株发生516 C→T占,各有1株MTB在905A→G、926 C→G发生突变,各种突变形式与北京基因型差异无统计学意义。结论:天津地区链霉素耐药株中以rpsL K43R突变形式最为常见,其次为K88R和rrs 513 A→C。在北京与非北京基因型MTB菌株中rpsL与rrs基因突变类型差异无统计学意义。更多还原

【Abstract】 Objective Determine the most common mutations of rspL and rrs genes in streptomycin(SM)-resistant Mycobacterium tuberculosis(MTB)isolated from Tianjin in order to select the target mutations to develop the rapid test for detecting SM-resistant tuberculosis.Investigating the whether the Beijing genotype associated with gene mutation types.Method 116 SM-resistant and 10 pan-sensitive MTB isolates were subject to rspL and rrs analysis by direct sequencing.Result 87.1%(101/116)SM-resistant isolates had missense mutations in rspL and rrs genes. Of these,70(60.3%)exhibited rpsL mutation at codon 43 AAG→AGG(K43R)and 15(12.9%) at codon 88 AAG→AGG(K88R).17 SM-resistant isolates had rrs mutations,these included 13(11.2%)strains at position 513 A→C,2 strains(1.72%)at position 516 C→T,1 strain at 905 A→G and 1 strain at 926 C→G.No association was found between mutation types and Beijing genotype.Conclusion rpsL K43R is the most common mutation among SM-resistant isolates,followed by rpsL K88R and rrs 513 A→C.Compared with non-Beijing genotype,rpsL and rrs mutations had no significantly associated with Beijing genotype.更多还原
