【摘要】 目的对固定剂量复合剂(FDC)与板式组合药治疗初治涂阳肺结核的效果进行对比和评价。方法通过中国生物医学文献光盘数据库和中国期刊全文数据库检索1994-2009年发表的关于固定剂量复合剂与板式组合药治疗初治涂阳肺结核效果对比的文献,按照纳入、排除标准选取后,采用RevMan5.0.23软件进行meta分析。结果共有6篇论著纳入meta分析,分析显示:基于FDC的化疗方案与板式组合药化疗方案的治愈率相比,固定效应模型OR值为1.91,95%CI(1.37,2.66),随机效应RR值为1.02,95%CI(1.00,1.06),提示应用FDC有可能提高初治涂阳患者的治愈率;对2月末痰菌阴转率[固定效应OR=1.16,95%CI(0.90,1.49)]、3月末痰菌阴转率[固定效应OR=1.27,95%CI(0.85,1.90)]和严重药物不良反应率[随机效应OR为1.14,95%CI(0.45,2.92)]等指标的影响没有统计学差异。结论现有文献结果支持FDC治疗初治涂阳肺结核患者效果比板式组合药更好,但此结论尚有待通过开展更多高质量的对照研究来进一步论证和检验。更多还原

【Abstract】 Objective To compare and access the treatment efficacy of new smear positive tuberculosis cases with fixed-dose combinations(FDC) and the blister packs.Methods All the control studies on the treatment efficacy of new smear positive cases with FDC and blister packs were identified from CBM and CNKI.The data from studies which meet the include criteria were analyzed with RevManS.0.23.Results 6 literatures were selected for meta-analysis.The cure rate of FDC may be higher than that of the blisters [fixed,OR= 1.91,95%CI(1.37,2.66);random,RR= 1.02,95%CI(1.00,1.06)].While for the conversion rate at the end of 2 month[fixed,OR=1.16,95%CI(0.90,1.49)]or 3 month[fixed,OR=1.27,95%CI(0.85,1.90)], there is no statistic difference were found between FDC and blisters,nor for the rate of severe adverse effects of anti-tuberculosis drugs[random,OR=1.14,95%CI(0.45,2.92)].Conclusion Existing data suggests that FDC might provide better cure rate than blisters.But this viewpoint still need further demonstration and testing by more control studies with good quality.更多还原
