【摘要】 目的:研究教学医院门诊医师处方习惯并采取持续干预措施降低不合理用药。方法 :20400张处方来源于2010年4月-2011年2月一年的门诊处方,每两月每个科室随机抽取200的门诊处方,按《处方管理办法》和《医院处方点评规范》实时进行处方点评,点评内容包括处方的基本要素、处方用药剂量、使用疗程、适应证、药物相互作用等信息。并将点评结果反馈于门诊医生。结果 :每张处方用药品种为1.84个,抗菌药物使用率为24.95%,合理处方百分率为89.95%。经过一年的点评并采取干预措施后,不合理处方从19.71%下降到5.88%。结论 :处方点评和干预措施显著的改善了处方质量。加强临床药师的药学的专业和临床专业知识学习将会进一步提高不合理处方的发现率。本文的结果为我院对深入处方点评工作实施奠定基础。更多还原

【Abstract】 Objecitive: To study physicians habits on the outpatient prescription in a teaching hospital and to suggest effective strategies for reducing irrational drug utilization.Method: A total of 20400 outpatient prescriptions screened randomly over one-year period (April 2010 to April 2011).The prescriptions of 200 was reviewed two monthly in each division.The prescriptions were evaluated by pharmacist according to "Administrative Rules on Prescriptions" and "Review on the management provisions of the hospital prescription".The prescriptions were analyzed for the essential elements of the prescription order and for the drug prescription proper (adequacy of dosage and duration of use,prescription and appropriateness of prescription to the diagnosis,and the interactions of drug-drug).The summarized results feed backed the physician.Result: The average number of drugs per prescription was 1.84(mean).The percentage of antibiotics agents accounted for 24.95%.Prescriptions of 89.95% were rational.The irrational prescriptions decreased step by step from 19.71% to 5.88% after one year period of evaluation on prescriptions and education to outpatients.Conclusion: Following continuous survey on prescription and education to physician,the quality of prescription was significantly improved.The pharmacists,who enhanced theirs knowledge and technique on pharmaceutical specialty and clinical specialty by study,increased the discovery rate of irrational prescription.The summary results established a foundation for the depth review of prescription.更多还原
