【摘要】 目的对我国现阶段基本药物临床使用和价格情况进行深入剖析,为新医改下的基本药物政策提供决策参考;方法以近年来国内文献研究为依据,并结合某市全部医院数据库,对基本药物使用和价格现状进行深层次分析;结果研究发现我国基本药物临床使用广泛,但存在部分品种短缺的现象,这集中表现为廉价药、限价药及罕见药短缺。数据显示我国总体药品价格水平持续下降,但人均费用水平居高不下,同时同一通用名药品不同厂商间价格差距较大;结论我们认为上述现象可能是由我国药品固定加成定价机制等多方面原因造成,我们建议在基本药物政策中以最高保险限价代替最高零售价格,取消医疗机构药品固定加成比例,以及实施预付制等付费方式等措施来解决上述问题。更多还原

【Abstract】 OBJECTIVE To analyze the utility and price of the essential medicine at present, and put forward suggestions for new medical reform. METHODS Based on the research of domestic literature and the insurance data, this paper explores the essential medicine’s application and price from different ways. RESULTS In clinical therapy essential medicine are used widely, but some species are of deficiency, especially cheap drugs, price fixing drugs, as well as orphan drugs. Data indicates drug price declined steadily while expenditure per capita keeps an upward trend, together with the same ingredient drugs made by different companies always have larger price discrepancy. CONCLUSION:We suppose the above situation is produced by the policy which allows the hospital to fix a stationary addition proportion into the drugs, so we suggest the policy of essential medicine include the maximum reimbursement may substitute the maximum retail price, and perspective payment system could be implemented instead of stationary addition proportion.更多还原
