【摘要】 目的:为重症患者提供全程药学监护,完善治疗方案,实现个体化用药。方法:结合患者合并症较多的情况,协同医生,根据药物的特点,对患者的抗感染、抗心衰、血糖控制、抗凝用药进行监护调整,并为患者提供用药教育。结果:临床药师参与下,患者病情好转出院。结论:临床药师对合并症较多、病情较重患者的药学监护,可以促进合理用药,减少药源性损伤,提高临床疗效。更多还原

【Abstract】 Objective:Supporting a severe case with pharmaceutical care and consummating the therapeutic schedule in order to accomplish personalized medicine.Methods:Considering the patient’s pathogenetic condition,we worked with the doctor to adjust the comprehensive treatment with anti-infection,anti-HF,glycemic control and anticoagulation.And conduct medicine education initiatively.Results:The patient got better then leave hospital.Conclusion:For patients with serious complications,our clinical Pharmacist can promote rational administration,decrease drug-induced injury and improve clinical effect.更多还原
