2型糖尿病患者血清TNF-α、sICAM水平的变化及与内生肌酐清除率的相关性分析

【摘要】 目的:探讨2型糖尿病(DM)患者血清肿瘤坏死因子(TNF)-α、可溶性细胞间黏附分子(sICAM)-1水平的变化及与内生肌酐清除率(Ccr)的相关性。方法:分别应用放射免疫分析方法、酶联免疫吸附法检测我院住院的122例2型DM患者血清sICAM-1、TNF-α水平,设正常对照组30例,比较二组之间血清sICAM-1、TNF-α水平,分析2型DM患者Ccr与血清sICAM-1、TNF-α水平的相关性。结果:2型DM患者血清sICAM-1、TNF-α水平显著高于正常对照组(178.25±33.34 ng/ml vs212.68±63.91 ng/ml,P<0.01;10.13±4.00 pg/ml vs 12.01±4.34 pg/ml,P<0.05)。调整了年龄、性别、DM病程后,2型DM患者血清TNF-α、sICAM-1之间呈显著正相关(γ=0.222,P=0.015)。2型DM肾病患者血清sICAM-1、TNF-α水平显著高于不合并DM肾病患者(200.34±56.05 ng/ml vs 228.74±70.22 ng/ml,P<0.05;11.15±2.69 pg/ml vs 13.13±5.68pg/ml,P<0.05)。2型DM患者Ccr与血清TNF-α、sICAM-1呈显著负相关(γ=-0.233、-0.185;P=0.010、0.041),调整了性别、年龄、DM病程、体质指数、血压、血脂、血糖控制情况后,Ccr仍与TNF-α、sICAM-1呈显著负相关(P=0.019、0.010)。结论:2型DM患者血清TNF-α、sICAM水平显著增高,其可能是2型DM患者Ccr下降的独立危险因素。

【Abstract】 Objective: To investigate the relationships between the changes of the levels of tumor necrosis factor(TNF)- alpha、soluble intercellular adhesion molecule(sICAM)-1 and endogenous creatinine clearance rate(Ccr) in type 2 Diabetes. Methods: The levels of sICAM-1、TNF-α of one hundred and twenty-two patients with type 2 diabetes were analyzed by radioimmunoassay and ELISA respectively. Thirty nomal person are in control group. To compare the levels of sICAM-1、TNF-α between two groups and analyze the relationships between the Ccr and TNF-α、sICAM-1 in type 2 Diabetes. Results: Compared to control group, the levels of sICAM-1、TNF-α were increased obviously(178.25±33.34 ng/ml vs 212.68±63.91 ng/ml,P<0.01;10.13±4.00 pg/ml vs 12.01±4.34 pg/ml,P<0.05). After adjusting the age,gender,the ascend duration of diabetes,it was found that sICAM-1 and TNF-α had a positive correlation(γ=0.222,P=0.015). Compared to type 2 Diabetes without nephropathy, the levels of sICAM-1、TNF-α of diabetic nephropathy were increased obviously(200.34±56.05 ng/ml vs 228.74±70.22 ng/ml,P<0.05;11.15±2.69 pg/ml vs 13.13±5.68 pg/ml,P<0.05). It was found that Ccr had a predominant negative correlation with TNF-α and sICAM-1(γ=-0.233、-0.185;P=0.010、0.041). After adjusting the age,gender,the ascend duration of diabetes,body mass index(BMI),blood pressure,lipid and blood glucose,Ccr still had a predominant negative correlation with TNF-α and sICAM-1(P=0.019、0.010). Conclusion: The levels of sICAM-1、TNF-α were increased obviously, which may be independent risk factors of the descent of Ccr in type 2 Diabetes.
