2型糖尿病患者不同自主神经病变检测方法的比较
The comparison between different methods to detect autonomic neuropathy in patients with T2DM

【摘要】 目的:研究2型糖尿病(T2DM)患者自主神经病变症状、心血管反射试验(cardio-vascular reflex test CRT)、排汗神经检测(欧米诺)诊断糖尿病自主神经病变(diabetic autonomic neuropathy DAN)的关系。方法:选取2005-08~2006-12南方医科大学附属南方医院门诊及住院的82例T2DM患者,分为无症状(DANS-)组和有症状(DANS+)组,CRT阴性(CRT-)组和CRT阳性(CRT+)组,40例健康人为对照组,进行排汗神经功能检测分析。结果:DANS+组、DANS-组及对照组间欧米诺变色时间差异显著(均P<0.05)。CRT-组、CRT+组、及对照组间欧米诺变色时间差异显著(P<0 01),欧米诺与CRT的阳性率分别为45.1%、43.9%,诊断符合率为93.9%。欧米诺变色时间与CRT各检测指标均显著相关。结论:采用欧米诺进行排汗神经功能检测可以早期发现T2DM患者自主神经病变。
【Abstract】 Objective:To investigate the relationship of the symptoms of automomic neuropathy in patients with T2 DM,CRT and perspiration-nerve examination to detect diabetic automomic neuropathy(AN).Methods:82 cases of outpatients and inpatients with T2 DM in Nanfag Hospital,the First Affiliated Hospital of Southern Medical University,from August,2005 to December,2006 were divided into the group of patients without AN symptoms(DANS- group),the group of patients with AN symptoms(DANS+ group),CRT positive group(CRT+ group) and CRT negative group(CRT- group),with40 healthy people as control group,to detect the function of perspiration nerve.Results:There were signifcant changes of color changing time between DANS+ group and control group,between DANS- group and control group,moreover,CRT+ group changed more significantly than CRT-group.The positive rate of Neuropad and CRT was 45.1%and 43.9%respectively.The diagnose accordance rate was 93.9%.Neuropad color changing time correlated to the each parameter of CRT significantly.Conclusion;AN in patients with T2 DM can be detected early with examination of perspiration-nerve function with Neuropad.
