【摘要】 [目的]回顾性分析胸腰椎骨折后路间接复位内固定早期治疗的疗效。[方法]对本院2000～2007年胸腰椎骨折病例115例伤后48 h,采用间接复位内固定和选择性椎管开窗减压治疗胸腰椎骨折,分析手术前后椎体前、后缘高度、Cobb角以及手术前后脊髓神经功能(按Frankel分级评定)的变化。[结果]经6～24个月,平均18.4个月的随访,术后椎体前、后缘高度较术前明显增高(96.3±2.4)%vs(51.1±11.3)%P<0.01,(98.1±0.9)%vs(80.2±3.6)%P<0.01,Cobb角术后较术前明显缩小(5.3±1.4)°vs(26.1±2.4)°P<0.01。术后91例患者神经功能有不同程度改善。[结论]胸腰椎骨折后路间接复位内固定早期治疗具有复位满意、手术创伤小、椎管容积恢复好、神经损伤恢复好的优点。更多还原

【Abstract】 [Objective]To retrospectively analyze the results for primary treatmen of thoracolumar fractures by indirect reductionand internal fixation.[Method]115 patients with thoracolumbar blowout fractures were treated by indirect reduction and internal fixation in 48 hrs after injuries.Both the heights of the anterior and the posterior edge of vertebral body and the Cobb angles of pre-operation and post-operation were measured The results of neurologic recovery after thoracolumbar fractures were recorded according to Frankel classification.[Results]All patients were followed up from six months to twenty four months with average of fourteen months.Compared with the pre-operative average height of both the anterior and the posterior edge of vertebral body,the post-operative average height of both the anterior and the posterior edge of vertebral body increased significantly(P<0.01).The Cobb angles decreased markedly(P<0.01). Among 91 patients with nerve injuries recovered in various degrees.[Conclusion]Primary treatment of the thoracolumbar fracture by by indirect reduction and internal fixation has the advantages such as satisfacto- ry reduction,minor surgical injury,fine reduction of canalis vertebralis and good neurologic recovery,in.更多还原
