e.max瓷贴面固位的单端全瓷翼板桥在前牙列区单颗牙缺失中的四年应用效果评价

Four-year clinical prospective evaluation of lithium-disilicate based veneer-retained cantilevered fixed dental prostheses for single anterior tooth loss

【摘要】 目的:通过制作e.max单端全瓷翼板桥,修复前牙列区单颗牙缺失,对其四年的临床应用效果进行评价,为其临床应用提供参考依据。材料和方法:患者于2005年10月-2011年2月之间就诊于中日友好医院口腔医学中心。共入组35人,其中男性患者13人,平均年龄40.3岁;女性患者22人,平均年龄43.2岁。其中,恢复上颌侧切牙15例,上颌第一前磨牙2例;恢复下颌中切牙8例,下颌侧切牙9例,下颌第一前磨牙1例;除一例瓷翼板设计在唇侧外,余全部设计在舌腭侧。翼板与桥体连接处龈距高度平均3 6mm,至少3.3mm(3.3-3.8mm),颊舌向平均宽度是2.8mm(2.5-3.1 mm),至少2.5mm。应用VariolinkⅡ作为树脂粘接剂,常规进行粘接。平均临床观察期为30月。结果:所有修复体无一例发生连接体折裂、饰瓷崩脱、基牙移位或松动。基牙和固位体无色素沉着,无继发龋发生。结论:应用e.Max瓷贴面固位的单端全瓷翼板桥修复前牙区单颖牙缺失可以实现微创修复效果,具有较好的临床预后,长期效果仍需要进一步观察。

【Abstract】 The purpose of this prospective study was to evaluate the clinical outcome of veneer-retained fixed dental prostheses(VRFDPs) with a cantilevered design made from lithium-disilicate based all-ceramic materials(IPS e.max Press).Methods:Patients were involved in the study during the year Oct.2005 to Feb.2011.A total of 35 patients were treated with this kind of prostheses after being instructed about the related information.Veneer preparation were perfomed according to the guidelines with all the retainer remained in the palatal/lingual surface except one case in the labial surface.Variolink II resin cement was used for the durable bonding according to the manufacture’ s instructions.Patients were recalled every year for a clinical examination to evaluate the restoration with regard to function and potential failure or something.The mean observation time was 30 months.Results:During the observation time,none of the prostheses failed without connector fracture,abutment moving and porcelain crack.No recurrent caries occurred.Conclusions: VRFDPs made from lithium-disilicate glass-ceramic can provide an esthetic and promising treatment option for single anterior tooth replacement.However.before this technique can be recommended for general use, clinical studies must confirm a positive long term outcome of these restorations.
