【摘要】 目的:评估根管治疗的转诊病例,分析转诊的主要原因。材料和方法:临床收集2010年10月至2010年11月转诊至四川大学华西口腔医院牙体牙髓病科行根管治疗的病例355例,从年龄、性别、教育程度、职业、全身疾病、转诊原因、牙位等方面进行统计,所得数据利用SPSS16.0软件进行卡方检验,检验水准为双侧α=0.05。结果:所有转诊病例中40-49岁的患者所占的比例最大,12%的患者患有全身性疾病,如高血压、糖尿病、冠心病、风湿性关节炎、抑郁症等。检查前根据患者主诉最常见的转诊原因是症状持续不消(28%),其次是根管钙化(26%),经临床检查诊断后最多见的转诊原因是根管钙化(30%),其次是根管再治疗(24%),差异有统计学意义(P<0.05)。所有病例中下颌第一磨牙最多见,尖牙和第三磨牙最少,差异有统计学意义(P<0.05)。结论:临床工作中,医师在治疗前应进行根管治疗难度评估,当难度超过医师自身的技术水平时,应建议患者进行转诊,以满足患者的最大利益和提高根管治疗的成功率。更多还原

【Abstract】 Objective To evaluate referral cases for endodontic treatment and analyze the reasons for referral.Materials and Methods 355 cases for endodontic treatment were referred to West China College of Stomatology from October 2010 to November 2010 .Basic information of patients such as age,sex,educational level,occupation,general diseases,the location of teeth and the reasons for referral were recorded.The data were analyzed by the statistical software SPSS 16.0.The chi-squared test was used to compare proportions,and the significance threshold for all tests was set at P<0.05.Results Of all the referred patients,the greatest number of patients(28%) are 40-49 yearsold,only a minority of patients(12%) suffered from the general diseases such as hypertension, diabetes mellitus,coronary artery disease,rheumatoidarthritis,depression and so on.Before examination the most reasons were persistent signs or symptoms(28%) and calcified root canals(26%).After clinical examination,the greatest percentage of reasons were calcified root canals(30%),the second was endodontic retreatment(24%). Mandibular first molars are most and canine and the third molars are the least.There are both significantly different(P<0.05).Conclusion In clinical practice,before treatment dentists should determine the degree of difficulty of treatment,when the degree exceeds the ability of a clinician,referral should be suggested in the best interests of the patients to improve the success rate of endodontic treatment.更多还原
