06锥度根管预备后根管不同充填技术对根管抗折性的影响
The effects of different filling technique for fracture resistance of root canal on 06 taper root canal preparation

【摘要】 目的:目前治疗牙髓病及根尖周病的主要及最佳治疗方法是进行根管治疗。但近年来发现过度的根管预备和充填压力可能造成无髓牙的牙根纵折,从而造成治疗失败。本实验通过大锥度机用镍钛锉预备根管后应用不同根管充填技术分析它们对根管抗折性的影响,为临床根管治疗技术提供一定的试验依据,尽可能降低根管折断的发生,提高根管治疗的成功率。方法:选择因拔除的下颌前磨牙120颗,随机分成A,B,C,D,组,A、B为实验组每组30颗,C、D为对照组每组30颗。经硬组织切片机截冠处理后,A、B、C采用HERO—642镍钛根管预备系统以冠向下预备成06锥度30#。A组采用AH-PLUS+牙胶尖进行侧方加压充填,B组采用AH-PLUS+牙胶尖进行三维根管充填,C组不充填,D组不进行任何处理。所有标本用磷酸锌水门汀封闭,37℃,100%湿度条件下备测。测试前将标本包埋于用丙烯酸树脂制作的一个高13mm,直径为15mm的圆柱形模具中,然后将试件置于万能测试机的下方平台,调节横梁使加载头的顶端正对牙根中心即磷酸锌水门汀封闭部位,以lmm/min的速度进行垂直加压,直到出现折裂。记录各组离体牙断裂时所受到的压力数值。所测数据采用SPSS软件进行方差及t检验统计学分析。结果:1)实验组(A,B)与对照组C之间进行抗折力比较存在显著差异(P<0.05);2)A组(冷侧压法)与B组(热垂直三维法)之间进行抗折力比较无显著差异(P>0.05);3)C组(预备不充填)与D组(不预备不充填)之间进行抗折力比较存在显著差异(P<0.05),D组的抗折力大于C组抗折力。结论:根管治疗会降低牙根的抗折力,但根管预备至06锥度后应用不同的根管充填方法对根管的抗折性无显著差异。
【Abstract】 Objective:The treatment of pulp disease and periapical disease is the main and the best treatment for root canal treatment.But in recent years found that over the root canal preparation and filling pressures may result in non-vital teeth in the root longitudinal fractures,resulting in treatment failure.The experimental machine used by large taper nickel titanium file preparation of the root canal after the application of technical analysis of different root canal filling root canal rupture of their impact.Root canal treatment for clinical trial technology to provide a basis,to minimize the occurrence of broken root canal,to improve the success rate of root canal therapy.Methods:Select pull out due to orthodontic mandibular premolars 120,randomly divided into A,B,C,D group.A,B for the experimental group,C,D as the control group.The crown of sections of hard tissue after treatment,A,B,C with HERO-642 nickel-titanium root canal preparation systems to prepare the crown down into the 30 # 06 taper.A group with AH-PLUS + gutta-percha lateral condensation filling the,B group with AH-PLUS + gutta percha root canal filling three-dimensional,C group were not filling,D group without any treatment.All specimens were sealed with zinc phosphate Ting water,37℃,100%humidity measurement equipment.Before testing the specimen was embedded in acrylic resin produced with a high 13mm,15mm diameter cylindrical mold,Then place the specimen under the platform of universal testing machine,adjust the beams to the top of the head is loaded on the root center that closed parts of zinc phosphate cement,To 1mm / min vertical speed of pressure until the fracture.Record breaking teeth from each group when the pressure value.The measured data using SPSS software for statistical analysis of variance and T test.Results:1) The experimental group(A,B) with the control group fracture resistance between C there were significant differences compared(P<0.05);2) A(cold lateral) and B(three-dimensional thermal vertical method) between the fracture resistance was no significant difference(P>0.05);3) C group(prepared without filling) and D(not prepared to not filling) Comparison between the fracture resistance were significantly different(P<0.05),D group than C group fracture resistance fracture resistance.Conclusion:Root canal therapy will reduce the root of the bending force,but taper root canal preparation to 06 taper after the application of different root canal filling techniques on the fracture resistance of root canals was no significant difference.
