显微超声技术处理根管中下段折断器械的效果评价
Evaluation of management of broken instrument in apical one third of root canal using the microsonic technique

【摘要】 目的评价显微超声技术处理根管中下段折断器械的临床疗效。方法对常规方法不能处理的,发生根管中下段器械折断的47个根管,共46颗患牙,应用显微超声技术进行根管再处理,统计治疗成功率,3、6、12个月后复查。结果 44例位于根管中下段的折断器械全部取出,无根管侧穿;1例由于根管弯曲度过大无法取出,2例折断器械虽取出但根管发生侧穿,侧穿病例行MTA穿孔修补,复查无1例患牙出现治疗失败。结论显微超声技术为处理根管内折断器械的理想手段,但需注意术前X线片等重要因素的分析,术后患牙发生根折的几率有待进一步研究。
【Abstract】 Objective:To evaluate the clinical effect of microsonic technique on management of broken instruments in apical one third of root canal.Methods:46 teeth with 47 broken instruments,which could not be successfully managed by conventional methods,were endodontically retreated with ultrasonic instruments under dental operating microscope.The success rate was calculated and cases were reviewed 3、6、12months after retreatment.Results:44 broken instruments were completely removed without perforation.1 broken instrument could not be removed because of the excessively curved root canal.2 broken instruments was removed but the root canals were perforated。They were repaired with Mineral Trioxide Aggregate(MTA) and obturated with thermplasticized gutta-percha.None of these teeth showed periapical deterioration in the subsequent visit. Conclusions:The combine technique using dental operating microscope and ultrasonic instruments is an effective method for removal of intracanal broken instruments.Carefully analysis of X-ray and other important factors should be paid attention to before operation. The incidence of root fracture after operation remains further investigation.
