【摘要】 目的:探讨口内入路颌下腺切除术可行性及手术安全性。方法:对70例颌下腺与颌外动脉的关系进行重新解剖及文献复习,为口内入路的颌下腺切除术提供解剖学基础。行口内入路的颌下腺切除术20例,含内窥镜辅助下口内入路5例。结果:67.1%(47/70)颌外动脉是走行于颌下腺腺体表面,分出2-4支滋养支供血腺体。15例口内入路的颌下腺切除术过程顺利,6例颌外动脉需术中结扎,未出现术中、术后大出血等并发症。内窥镜有利于口底区血管的结扎和止血。结论:只要严格掌握适应症,妥善处理颌外动脉,口内入路的颌下腺切除术是安全可靠,内窥镜可增加术野的清晰度。更多还原

【Abstract】 Objective:To investigate the feasibility and safety of intraoral submandibular gland excision.Method Analyze the relationship between external maxillary artery and submandibular gland by dissecting 70 cases and review the conference concert,and offer a reliable anatomical base for 20 cases of intraoral submandibular gland excision including 5 cases under endoscope assisting. Results 67.1%(47/70)The external maxillary artery freads on the surface of gland submandibular,and its 2 to 4 branches provide nutrition for the gland.15 cases of intraoral submandibular gland excision were smooth,and the results of the all cases were effective and satisfied,without massive hemorrhage during or post-operation.External maxillary artery need to be ligated Only in 6 cases during operation.Endoscope and ultrasound knife were behave for vascular vessels ligating And hamostasis.Conclusion:Intraoral submandibular gland excision is safe and feasible as long as indications stricdy controlled and the external maxillary artery coped well with.Endoscope is good for operating vision in deep region.更多还原
