【摘要】 目的评价护士主导的全科团队家访服务对社区高龄居家老人生活质量的影响。方法采用投掷硬币法将144名高龄居家老人按照居住街道分为干预组(74例)和对照组(70例)。干预组接受护士主导的全科团队家访服务,包括健康评估、健康问题排序及慢性病指导、心理护理、日常生活指导和居家安全宣教等个体化家访服务;对照组接受常规社区卫生服务。比较两组干预前、干预后3个月、6个月生活质量。结果两组高龄老人生活质量的生理领域、环境领域、心理领域、社会领域评分比较,差异有统计学意义(P<0.05,P<0.01);干预组在干预3个月、6个月时各领域得分均较干预前显著提高,而对照组无明显变化。结论护士主导的全科团队家访服务能显著改善高龄居家老人的生活质量,此服务模式具有可操作性和实用性。更多还原

【Abstract】 Objective To evaluate the effects of nurse-led multidisciplinary home visiting program to the quality of life among community-dwelling oldest old. Methods 144 aged over 80 years old people were randomly divided into the intervention group (n=74) and the control group (n=70). The intervention group received the nurse-led multidisciplinary team home visiting, such as health assessment, health problem sorting, chronic disease guidance, psychological nursing and daily life guidance; The control group received usual community health service. The QOL changes were compared by the repeated-measure ANOVA. Results At 3 months and 6 months after the intervention,the physical domain, psychological domain, social relations and environment domain of QOL were significantly different between the two groups(P<0.05,P<0.01). The QOL in the intervention group displayed an obvious upward trend compared with the control group. Conclusion The nurse-led multidisciplinary home visiting program can significantly improve the quality of life among community-dwelling oldest old. It is proved to be feasible and practical,which is worth to be spread in community.更多还原
