医院消毒监测金黄色葡萄球菌主要毒力基因和耐药基因研究

【摘要】 目的研究医院分离金黄色葡萄球菌分离株的毒力基因和耐药基因携带情况,为加强医院感染控制提供科学依据。方法采用分离培养与鉴定方法和多重基因扩增技术对金黄色葡萄球菌分离株进行了毒力基因和耐药基因检测,同时进行药敏试验。结果在医院采集的5088份样品中,分离出25株金黄色葡萄球菌,阳性率为0.49%,其中耐甲氧西林菌株占12%。检测出主要毒力基因SEA-E携带率为52%,ETA携带率为12%,PVL,ETB和TSST-1等携带率均比较低。检出耐药基因主要是tetM,携带率为44%,erm携带率为24%.临床分离的金黄色葡萄球菌对常用12种抗菌药物都不同程度耐药,主要耐药基因和相应耐药表型结果基本一致。结论医院分离的金黄色葡萄球菌携带较多的毒力因子和耐药因子,MRSA具备多重耐药性,耐药表型和耐药基因检测结果相一致。

【Abstract】 Objective:To understand the carrying case of mainly virulence genes and resistance genes for staphylococcus aureus from hospital disinfection monitoring in zhaoqing, providing scientific basis to strengthen the hospital infection control.Methods:According to isolation and identification methods,and the multiple PCR technology,the mainly virulence genes and resistance genes of staphylococcus aureus were analyzed,and the antimicrobial susceptibility test was also done by Kirby-Bauer disc diffusion method, simultaneously.Results:25 strains of staphylococcus aureus were separated from 5088 copies of samples,making the positive late comes to 0.49%,and the MRSA strain reach 12.00% among.The virulence genes were mainly SEA-E,and ETA,the carrying rate were 52.00% and 12.00%,respectively;and the PVL,ETB,TSST-1 were lower.The resistance genes were mainly tetM and erm,the rates were 44.00%and 24.00%,respectively.The isolated strain have different drug-resistantce to 12 species of antimicrobial.The results of mainly resistance genes detection were almost agree with corresponding drug-resistant phenotype test. Conclusion:The isolated staphylococcus aureus strain from hospital,were carring much more virulence and drug-resistant factors,and the MRSA strains have multiple drug resistance. Gene detection and drug-resistant phenotype results were consistent.
