产妇血和新生儿脐带血重金属水平相关性分析及其影响因素

【摘要】 目的了解深圳市产妇全血及新生儿脐带血中12种重金属元素(镉、铅、铜、铬、锰、钴、砷、硒、钼、锑、汞和铊)的负荷水平及其相关影响因素分析。方法选择2010年3月~4月间在深圳某医院住院分娩、年龄21~40岁的产妇109名,分别采集产前静脉血和分娩时胎儿脐血,并对产妇进行家庭社会环境和健康问卷调查。采用电感耦合等离子体质谱法及电感耦合等离子体发射光谱法测定全血中镉、铅、铜、铬、锰、钴、砷、硒、钼、锑、汞和铊的含量。结果有1例产妇血镉超标,其他重金属浓度均未超标。脐带血中汞、铬、锰、钼浓度高于母血,铅、镉、铜、硒、铊、锑、钴浓度则低于母血(P<0.05)。新生儿脐带血中铅、汞、镉、砷、锰、硒、钼、铊、锑、钴元素浓度与母血中同种元素浓度显著正相关(P<0.01),并且全血中铅和镉浓度具有正相关关系,砷、汞、硒三种元素浓度显著正相关(P<0.05)。孕妇吸烟状况与母血和脐血中钼浓度有相关性,孕妇海产品食用情况可影响母血镉浓度(P<0.05)。而新生儿生长发育状况与新生儿脐带血中重金属浓度无关(P>0.05)。结论深圳地区产妇血和脐带血中重金属暴露处于较低水平,血镉超标率远低于大多数其它国家和地区。胎盘对重金属缺乏有效屏障作用。

【Abstract】 Objective To determine concentrations of cadmium(Cd),lead(Pb),copper(Cu), chromium(Cr),manganese(Mn),cobalt(Co),arsenic(As),seleniurn(Se),rnolybdenum(Mo),antimony(Sb), mercury(Hg) and thallium(Ti) in maternal and umbilical cord blood and their influencing factors. Methods Totally 109 healthy pregnant women were selected as study subjects,maternal and cord blood samples were collected.The levels of 12 kinds of heavy metal in whole blood samples were determined with ICP-MS and ICP-AES.Results Only one example maternal blood cadmium level out of limits,the other heavy metal concentration were not overweight.Concentrations of Hg,Cr,Mn and Mo in the cord blood were significantly higher than that in the maternal blood,but concentrations of Pb,Cd,Cu,Se,Ti, Sb and Co in the cord blood were significantly lower than that in the maternal blood(P<0.05).There were significant correlations in the concentrations of Pb,Hg,Cd,As,Mn,Se,Mo,Ti,Sb and Co between the maternal and cord blood(P<0.01).The concentrations of Pb and Cd were significant correlated in maternal and umbilical cord blood.There were significant correlations among the concentrations of As, Hg and Se either in maternal or in umbilical cord blood(P<0.05).The concentration of Mo in the maternal or cord blood correlated significantly with smoke,and the concentration of Cd in the maternal blood correlated significantly with seafood intake(P<0.05).There was no significance correlation between heavy metal in umbilical cord blood and the development of the newborn(P>0.05).Conclusion The exposure of the heavy metal in maternal and umbilical cord blood in Shenzhen is lower than other country and area.There were not effectively barricaded by placenta
