β-淀粉样蛋白斑块显像剂131I-IMPY脑显像诊断Alzheimer病

【摘要】 目的:探讨β-淀粉样蛋白斑块显像剂131I-2-(4’-二甲基氨基苯基)-6-碘咪唑并[1,2-α]吡啶(IMPY)脑SPECT显像对阿尔茨海默病(Alzheimer’s Disease,AD)的诊断价值。方法:对24例AD患者(男7例,女17例,年龄48~79岁)及14例健康志愿者(男6例,女8例,年龄42~67岁)静脉注射131I-IMPY 37~74 MBq后2~3 h行脑SPECT断层显像,用感兴趣区(ROI)技术半定量分析方法,计算脑OM线横断面、矢状面各脑区/小脑(CB)放射性摄取比值(Rcl/cb),并进行两样本间t检验。结果:轻度认知损害期(MCI)患者中,顶叶和颞叶及额叶与小脑摄取比值增加,其与正常志愿者相比差异有统计学意义(t=1.396 7~2.875 7,P均<0.05),而基底部和枕叶与小脑摄取比值与正常志愿者相比差异没有统计学意义(t=0.780 5~1.434 4,P均>0.05);中晚期AD患者中,顶叶、颞叶、额叶、基底部和枕叶与小脑摄取比值均有不同程度的增加,其与正常志愿者相比差异均有统计学意义(t=2.100 1~6.278 9,P均<0.05),其中顶叶、颞叶、额叶与小脑摄取比值与MCI患者相比差异没有统计学意义(t=0.131 6~0.980 6,P均>0.05),基底部和枕叶与小脑摄取比值增加,其与MCI患者相比差异均有统计学意义(t=2.085 0~3.677 2,P均<0.05);各实验组左侧各脑叶皮质摄取与小脑比值较右侧摄取略偏高,但二者差异没有统计学意义(t=0.127 3~0.557 1,P均>0.05)。结论:131I-IMPY脑SPECT显像是诊断AD的有效方法,有助于AD的早期诊断。

【Abstract】 Objective To evaluate the diagnosic value of β -amyloid plaques imaging agent[1311]-2-(4’-dimethylaminop henyl) -6-iodoimidazo[l,2-α]pyridine(131I-IMPY)SPECT imaging in early Alzheimer’s Disease.Methods 24 cases of AD(7 males,17 females,aged 48~79 years)and 14 normal(6 males,8 females,aged 42~67 years)control subjects were selected for this study.131I-IMPY SPECT imaging was carried out 2-3 h post injection.131I-IMPY uptake defined as the ratio of each brain gyrus and cerebellum uptake on fixed region of interest(ROI)(Rcl/cb)was calculated.Comparative analysis between the two groups was carried out using t-test.Results In patients with early ADC MCI),131I-IMPY was increased in parietal gyrus,temporal gyrus and frontal gyrus compared with normal control group and it were found to be statistically significant(t=1.396 7~2.875 7,all P<0.05),whereas basel ganglia and occipital gyrus Rcl/cb compared with normal control group but without statistically significant difference(t=0.7805~1.4344,all P>0.05).In patients with AD,increase in~(131)I-IMPY were observed in parietal,temporal,occipital lobes and basal ganglia compared with normal control group and it were found to be statistically significant(t =2.100 1~6.278 9,all P <0.05),and parietal,temporal and frontal lobes Rcl/cb compared with MCI group but without statistically significant difference(t=0.131 6~0.980 6,all P>0.05),and~(131)I-IMPY was increased in occipital lobes and basal ganglia compared with MCI group and it were found to be statistically significant(t=2.085 0~3.677 2,all P<0.05).~(131)I-IMPY was lightly increased in each brain of left side gyrus compared with right but without statistically significant difference(t =0.127 3~0.557 1,all P >0.05).Conclusions~(131)I-IMPY SPECT Imaging was helpful for early diagnosis of AD.
