肿瘤转移抑制基因KISS-1在甲状腺癌组织中表达和预后的研究

【摘要】 目的检测KISS-1蛋白、MMP-2、MMP-9及Ki67在甲状腺癌中的表达,探讨四者在甲状腺癌发生发展过程中的作用。方法采用免疫组织化学SP法检测10例正常甲状腺组织、12例结节性甲状腺肿,、13例甲状腺腺瘤,68例甲状腺癌组织KISS-1蛋白、MMP-2、MMP-9及Ki67的表达,分析四者在不同临床病理特征中表达的意义及它们之间的相关性。结果甲状腺癌组KISS-1蛋白阳性表达率(36.7%)明显低于正常甲状腺组织、结节性甲状腺肿和甲状腺腺瘤组(分别为100.0%、83.3%、61.5%;P<0.01),并且KISS-1蛋白低表达与甲状腺癌的临床分期和淋巴结转移有关(P<0.05),与甲状腺癌患者的年龄、病理类型无关(P>0.05)。甲状腺癌组和结节性甲状腺肿和甲状腺腺瘤组MMP-2阳性表达率(分别为77.5%,53。8%、66.7%)明显高于正常甲状腺组(10.0%;P<0.01),并且MMP-9高表达与甲状腺癌的临床分期和淋巴结转移有关(P<0.05),与甲状腺癌患者的年龄、病理学类型无关(P>0.05)。甲状腺癌组和结节性甲状腺肿和甲状腺腺瘤组MMP-9阳性表达率(分别为82.3%,69.2%、58.3%)明显高于正常甲状腺组(20.0%;P<0.01),并且MMP-9高表达与甲状腺癌的临床分期和淋巴结转移有关(P<0.05),与甲状腺癌患者的年龄、病理学类型无关(P>0.05)。甲状腺癌组Ki67阳性表达率(88.2%)明显高于正常甲状腺组和结节性甲状腺肿和甲状腺腺瘤组(10.0%、30.7%、35.7%;P<0.01),并且Ki67高表达与甲状腺癌的临床分期有关(P<0.05),与甲状腺癌患者的年龄、病理学类型和淋巴结转移无关(P>0.05)。甲状腺癌中KISS-1蛋白与MMP-2、MMP-9表达呈显著负相关(r=-0.399,P<0.01)。结论 KISS-1蛋白、MMP-2、MMP-9及Ki67与甲状腺癌的发生发展有关,联合检测四者的表达可能作为评估甲状腺癌预后和指导临床靶向治疗的重要指标。

【Abstract】 Objective To investigate the expression of KISS-1 protein,MMP-2,MMP-9 and Ki67,and their relationship in carcinoma of the thyroid.Methods The expression of KISS-1 protein,MMP-2、MMP-9 and Ki67 was examined by a immunohistochemical assay(SP) in lOcases of normal thyroid tissue 12cases nodulcer goiter of thyroid,13cases of thyroid follicular adenoma,68 cases of thyroid carcinoma To analyze the relationship between them and the characteristics of clinical pathology of thyroid carcinoma. Results It was found that the positive rate of KISS-1 protein expression in thyroid carcinoma group (25.0%) was significantly less than thyroid nodulcer goiter,thyroid follicular adenoma and normal thyroid tissue group(77.5,53.8%,100%)(P <0.01).Thelow expression of KISS-1 protein was correlated with clinical stage and lymph node metastasisof thyroid carcinoma P < 0.05),but was not correlated with age of patients tand histogical grade of thyroid carcinoma.The positiv rate of MMP-2 and MMP-9expression in thyroid carcinoma and thyroid nodulcer goiter,thyroid follicular adenoma group(79.17%,55.00%) was significantly higher than normal thyroid tissue group(%)(P < 0.01 ).The high expression of MMP-2, MMP-9was correlated with clinical stage and lymph node metastasisof thyroid carcinom(P <0.05),but was not correlated with age of patients and histogical grade of thyroid carcinoma.The positive rate of Ki67expression in thyroid carcinoma and thyroid nodulcer goiter,thyroid follicular adenoma group (85.42%,60.00%) was significantly higher than normal thyroid tissue group(13.33%)(P < 0.01 ). The high expression of Ki67was correlated with clinical stage(P <0.05),but was not correlated with age of patients,histogical grade and lymph node metastasisof thyroid carcinoma.The expression of KISS-1 protein is negatively correlated withMMP-2 andMMP-9 in thyroid carcinoma(r = - 0.399,P < 0.01 ). Conclusion The expression of KISS-1 protein,MMP-2,MMP-9 and Ki67 is associated with the occurrence and development of thyroid carcinoma.Combined analysis of these three proteins may be an important index to judge prognosis and direct clinical targeted therapy of thyroid carcinoma.
