重庆市与西藏林芝地区碘缺乏病高危地区病情调查结果对比分析

【摘要】 目的调查重庆市与西藏林芝地区碘缺乏病高危地区病情现状,为制定防控方案提供依据。方法国家制定方案并统一抽样,定性检查居民食用盐碘,以触诊和B超法同时检查8~10岁儿童甲状腺,定量检测儿童和育龄妇女尿碘,搜索克汀病病例,用x2检验和t检验进行统计分析。结果重庆市调查2县,西藏林芝地区调查4县,居民碘盐覆盖率,重庆为98.82%(336/340),林芝地区为61.28%(607/905),重庆显著高于林芝地区(P<0.01);儿童甲状腺肿大率,触诊法和B超法重庆市分别为9.27%(89/960)和8.34%(61/731),林芝地区分别为7.80%(102/1308)和5.53%(69/1248),两地之间触诊法无显著差异(P>0.05),B超法重庆显著高于林芝(P<0.01);尿碘中位数,儿童和育龄妇女重庆分别为319.15μg/L和248.42μg/L,林芝地区分别为189.81μg/L和121.25μg/L,均以重庆为高(P<0.01);重庆和林芝地区均未发现新发克汀病病例。结论重庆和林芝地区碘缺乏病高危地区儿童甲状腺肿大率降至10%以下,未发现新发克汀病,防治成效显著。但重庆市补碘过量,林芝地区补碘不足,同时也存在碘过量。应综合防治,科学补碘。

【Abstract】 Objective Investigate the iodine deficiency disease(IDD) situation of Chongqing and Linzhi,provide scientific basis for IDD control and prevention. Methods Qualitatively detect resident’s diet idolized salt.Inspect thyroid by B ultrasound and palpation for children which fall into 8 to 10 age group.Quantitative detect the urinary iodine of children and childbearing age women.Search cretin cases.All the samples are collected according to national programs in both the two areas.x2 Test and t-test were used for statistical analysis.Results The investigation was conducted in 2 counties in Chongqing and 4 counties in Linzhi.The coverage of iodized salt in Chongqing,which is 98. 82%,is significantly higher than that in Linzhi,which is 61.28%.(P<0.01).Goiter rate of children in Chongqing is 9.27%by palpation and 8.34%by B ultrasound,while,Goiter rate of children in Linzhi is 7.8%by palpation and 5.53%by B ultrasound.Goiter rate of children by palpation has no significant difference between Chongqing and Linzhi.But Goiter rate of children by B ultrasound in Chongqing is higher than that in Linzhi(P<0.01).In Chongqing,the urinary iodine medians are 319.15μg/L for children and 189.81μg/for childbearing women,and in Linzhi,they are 248.42μg/L for children and 121.25μg/L for childbearing women.The urinary iodine medians in Chongqing both for children and women are higher than those in Linzhi.(P<0.01).There is no new cretin case occurred both in the two areas.Conclusion Goiter rate in Chongqing and Linzhi decreased to under 10%.No new cretin case occurred in the two areas.It can be concluded that the work of control and prevention is effective.But population iodine is excess in Chongqing.And in Linzhi,both insufficient and excess iodine exist.IDD should be controlled and prevented by comprehensive methods and iodine should be added scientifically.
