【摘要】 五十年代至今,我国医院建设取得了巨大成就,但早期的医院建筑已经无法满足当下的社会需求,医院建筑改扩建已经成为医院建设常态。医院始终以一种动态的形式存在,适应时代变化和不断更新的社会需求。目前医院建筑规模扩大,医护数量增加、医疗设施明显改善,但是医院依旧普遍存在空间拥挤、患者就诊时间长、满意度下降等现象,由此可见寻找有效的建筑空间优化方法,提升空间使用效率是当下医院建设亟待解决的问题。本文选取大型综合医院急诊空间为研究对象,以精益理念为理论指导,为急诊空间的优化设计提供科学决策。本文主要分为以下几个部分:第一章,阐述当下大型综合医院的现状和精益理念在建筑效率提升方面的作用,明确大型综合医院急诊空间为研究对象,界定其概念和范围。并且对大型综合医院急诊空间和精益理念的国内外研究现状进行综述,确定研究内容、研究方法和基本框架;第二章,采用文献研究和学科交叉的方法,对大型综合医院急诊科、精益理念和优化设计三部分理论进行梳理;第三章,总体阐述调研设计。预调研采用网络问卷的形式,了解当前大型综合医院急诊空间存在的问题,在此基础上筛选出正式调研的研究关键点;正式调研采用现场调查、跟踪观察、问卷调查等方法,在深圳市的22家三级甲等大型综合医院中选取华中科技大学协和深圳医院、北京大学深圳医院、深圳市人民医院,这三家医院作为具体研究对象,并明确调研实施方法及步骤;第四章,对急诊空间的调研结果进行统计和分析。主要分析功能布局变化、设施布点、就诊各环节时间、就诊流线,找出目前急诊空间存在的浪费和必要非增值内容;对满意度问卷结果进行信度分析、相关性分析、回归性分析和均值分析,以此了解患者及家属对于急诊空间的使用偏好;第五章,对调研的三家医院的急诊空间进行优化设计,并从平面布局、行为流线、导视标识和服务设施四个方面给出具体的优化策略。本文基于大量的研究探讨大型综合医院急诊空间的优化设计策略,延伸了精益理论在急诊空间的方法与应用,为未来开展医院空间精益设计提供理论依据和设计参考,进一步提升医院建筑空间的使用效率。更多还原

【Abstract】 Since the 1950s,hospital construction in China has made great achievements.However,the early hospital construction has been unable to meet the current social needs,hospital building renovation and expansion has become the normal hospital construction.Hospitals always exist on a dynamic form to adapt to the changing times and the constantly updated social needs.At present,the scale of hospital construction has expanded,the number of medical care has increased,and medical facilities have been significantly improved.However,there are still many problems of hospitals,such as crowded space,long time for patients to see a doctor,and decreased satisfaction.Therefore,it can be seen that finding effective methods of building space optimization and improving the efficiency of building space use is urgent problems to be solved in hospital construction.This paper selects the emergency space of large general hospital as the research object,and takes the lean concept as the theoretical guidance to provide scientific decision-making for the optimal design of emergency space.This paper is mainly divided into the following parts.The first chapter expounds the current situation of large general hospitals and the role of lean concept in improving construction efficiency,defines the emergency space of large general hospitals as the research object,and defines its concept and scope.It also summarizes the research status of emergency space and lean concept in large general hospitals at home and abroad,and determines the research content,research methods and basic framework.In the second chapter,using the methods of literature research and interdisciplinary,combs the theories of emergency department,lean concept and optimal design of large general hospital.In the third chapter generally expounds the research design.The presurvey adopts the form of network questionnaire to understand the problems existing in the emergency space of large general hospitals,and on this basis,clarify the research key points of formal survey.The formal investigation adopts the methods of on-site investigation,follow-up observation,questionnaire survey and so on.Among the 22 Grade-Three Level-A comprehensive hospitals in Shenzhen,three hospitals,Union Shenzhen Hospital,Peking University Shenzhen Hospital and Shenzhen People’s hospital were selected as specific research objects,and the implementation methods and steps of the research were clarified.In the fourth chapter makes statistics and Analysis on the research results of emergency space.Analyze the function layout,facility layout,time of each link of treatment and treatment flow line,and find out the waste and necessary nonvalueadded content of the current emergency space.Reliability analysis,correlation analysis,regression analysis and mean analysis were carried out to understand the patients’ preference for emergency space;In the fifth chapter optimizes the emergency space of the three hospitals,and gives specific optimization strategies from four aspects: space layout,behavior streamline,guidance signs and service facilities.Based on a large number of studies,this paper discusses the optimal design strategy of emergency space in large general hospitals,extends the method and application of lean theory in emergency space,provides theoretical basis and design reference for the lean design of hospital space in the future,and further improves the use efficiency of hospital buildings.更多还原
