【摘要】 目的:观察电子捻针仪联合电针治疗中风后尿潴留的临床疗效,探索电子捻针仪在疾病治疗中的作用。方法:依据门诊及病房患者先后就诊顺序,将符合课题纳入标准的中风后尿潴留患者按1～60排序,满60名患者为止。再用电脑随机分成两组,每组各30例。一组为治疗组,另一组为对照组。两组针刺选穴相同,在中风病常规针刺治疗的基础上,头针穴位选取百会和双侧足运感区;电针穴位选取中髎和次髎,其余治疗尿潴留的穴位为:肾俞(双侧)、膀胱俞(双侧)、秩边(双侧)。对照组选用普通针刺+电针治疗,治疗组选用电子捻针仪+电针治疗,二者的区别在于是否用电子捻针仪捻转百会和足运感区。观察治疗前后两组尿潴留患者拔除尿管时间、B超下残余尿量和膀胱功能积分的变化情况,并对两组的疗效进行评定。结果:1.两组患者治疗前在性别、中风类型、年龄及病程比较方面,不存在明显性差异(P>0.05),具有可比性。2.组内进行比较,治疗前后两组尿潴留患者B超下残余尿量和膀胱功能积分的比较(P<0.01),均有极显著统计学意义。3.组间治疗后比较,通过两组尿潴留患者拔除尿管时间、B超下残余尿量和膀胱功能积分的比较,治疗组明显优于对照组(P<0.05),具有显著差异。4.治疗结束后,采用中医学癃闭疗效评定标准及现代医学尿潴留疗效评定标准评价,电子捻针仪配合电针治疗的总有效率为93.33%,普通针刺配合电针的总有效率为83.33%,两组疗效差异有显著的统计学意义(P<0.05)。结论:1.电子捻针仪联合电针及普通针刺联合电针均能有效改善中风后尿潴留症状,有利于提高中风患者的生活质量。2.通过疗效观测,电子捻针仪联合电针组的总体疗效明显优于普通针刺联合电针组,临床疗效显著。更多还原

【Abstract】 Objective: To observe the clinical efficacy of electronic twisting needle instrument combined with electroacupuncture in the treatment of post-stroke urinary retention and to explore the role of electronic twisting needle instrument in disease treatment.Method: Patients with post-stroke urinary retention who met the subject inclusion criteria were sorted by 1-60 according to the order of outpatient and ward patient visits,until 60 patients were full.The patients were then randomly divided into two groups by computer,with 30 cases in each group.One group was the treatment group and the other group was the control group.The acupuncture points selected were the same in both groups,and on the basis of the conventional acupuncture treatment for stroke disease,the head acupuncture points were selected from Baihui and bilateral foot transport sensory area;the electroacupuncture points were selected from Zhongliao and Ciliao,and the remaining acupuncture points for the treatment of urinary retention were: Shenshu(bilateral),Bladdershu(bilateral),and Zhibian(bilateral).In the control group,ordinary acupuncture + electro-acupuncture was used,and in the treatment group,electronic twisting needle apparatus +electro-acupuncture was used.The difference between the two groups was whether or not to twist Baihui and foot transport sensory area with the electronic twisting needle apparatus.The time of catheter removal,residual urine volume and bladder function score under ultrasound were observed in the two groups before and after treatment,and the efficacy of the two groups was evaluated.Result:1.There was no significant difference between the two groups in terms of gender,stroke type,age and duration of disease before treatment(P > 0.05),and they were comparable.2.Comparison was performed within the group,and the comparison of residual urine volume and bladder function score under ultrasound between the two groups of patients with urinary retention before and after treatment(P< 0.01)were highly significant statistically.3.After the comparison between groups after treatment,by comparing the time of catheter removal,residual urine volume under ultrasound and bladder function score between two groups of patients with urinary retention,the treatment group was significantly better than the control group(P < 0.05),with significant differences.4.At the end of the treatment,the total effective rate of electronic twisting needle instrument with electro-acupuncture treatment was 93.33%,and the total effective rate of ordinary acupuncture with electro-acupuncture was 83.33% when evaluated by the efficacy assessment standard of retention of urine in Chinese medicine and the efficacy assessment standard of urinary retention in modern medicine,and the difference in efficacy between the two groups was statistically significant(P < 0.05).Conclusions:1.Both electronic twisting needle apparatus combined with electro-acupuncture and ordinary acupuncture combined with electro-acupuncture can effectively improve the symptoms of urinary retention after stroke,which is conducive to improving the quality of life of stroke patients.2.Through the efficacy observation,the overall efficacy of the electronic twisting needle instrument combined with electro-acupuncture group was significantly better than that of the general acupuncture combined with electro-acupuncture group,and the clinical efficacy was significant.更多还原
