【摘要】 目的:通过总结我师的多年研究经验和临床实践,有目的地设计临床观察性研究,对于肾虚血瘀型POI的患者进行针刺配合中药自拟方治疗,系统评价该治法对肾虚血瘀型早发性卵巢功能不全的临床治疗效果。方法:择符合标准的肾虚血瘀型早发性卵巢功能不全患者30例,运用针刺配合中药自拟方来进行治疗3个月,通过对比患者自身症状和血清FSH值在治疗前后的改变,用积分法对患者治疗前后中医证候疗效进行判定和比较。结果:针刺配合中药自拟方治疗30例POI患者综合疗效结果:无痊愈患者,显效5人,占16.67%,有效22人,占73.3 3%,无效3人,占1 0%,总有效率为8 3.3 3%。将患者治疗前与治疗后的中医症状积分进行比较,治疗前为34.2±3.19,治疗后为17.63±6.94,疗前与疗后对比有显著性差异(P<0.05)。患者在治疗后的各项中医症状均有改善,如月经后期、闭经、月经量少、腰酸膝软、头晕耳鸣等症状有明显的减轻,其他症状也有所缓解。并且通过与患者年龄和患者病程比较我们可以得知患者年龄越小、病程越短治疗效果越好。血清中性激素的水平也有所变化,血清FSH值在治疗前为49.23±8.83(IU/L),治疗后为21.52±13.34(IU/L),经分析显示,血清FSH值水平在治疗后明显下降,具有统计学意义(P<0.05)。结论:通过对肾虚血瘀型POI患者进行针刺配合口服中药自拟方的疗效观察发现,治疗取得了良好的临床疗效,而且临床疗效与患者的年龄和病程密切相关,年龄越小、病程越短的其治疗效果越好。更多还原

【Abstract】 Objective:Through summing up my teacher’s years of researc h experience and clinical practice,we purposefully designed clini cal observational research.We treated the patients with POI of ki dney deficiency and blood stasis type by acupuncture combined w ith self-made Chinese medicine prescription,and systematically e valuated the clinical therapeutic effect of this treatment on early-onset ovarian dysfunction of kidney deficiency and blood stasis t ypeMethods:Thirty patients with early-onset ovarian insufficienc y of kidney deficiency and blood stasis type were selected and tre ated with acupuncture combined with self-made prescription of tr aditional Chinese medicine for three months.The curative effect of TCM syndromes before and after treatment was judged and com pared by integral method through the changes of patients’own sym ptoms and serum FSHResults:Acupuncture combined with self-made prescription of traditional Chinese medicine in the treatment of 30 patients with POI of kidney deficiency and blood stasis type showed that 5 cas es were markedly effective,accounting for 16.67%,22 cases were effective,accounting for 73.33%,3 cases were ineffective,accou nting for 10%,and the total effective rate was 83.33%.The score s of TCM symptoms before and after treatment were compared.Th e scores before and after treatment were 34.2±3.19 and 17.63±6.94.There was a significant difference between before and after trea tment(P<0.05).After treatment,all the symptoms of traditional Chinese medicine were improved,such as late menstruation,amen orrhea,less menstrual volume,lumbar acidity,knee weakness,diz ziness and tinnitus,and other symptoms were alleviated.And by c omparing with the patient’s age and course of disease,we can kno w that the younger the patient’s age,the shorter the course of dis ease,the better the therapeutic effect.Serum levels of sex hormo nes also changed.Serum FSH values were 49.23±8.83(IU/L)befor e treatment and 2 1.5 2±1 3.3 4(IU/L)after treatment.Analysis show ed that serum FSH levels decreased significantly after treatment,with statistical significance(P<0.05)Conclusion:Through the observation of the curative effect of acupuncture combined with oral Chinese medicine prescription on POI patients with kidney deficiency and blood stasis,it was foun d that the treatment achieved good clinical efficacy,and the clini cal efficacy was closely related to the age and course of the patie nts.The younger the age and the shorter the course of the disease,the better the therapeutic effect.更多还原
