住院精神障碍患者支持性就业服务的策略探究

【摘要】 近年来,精神疾病的患病率不断提高,职业康复是推进患者康复的一个重要部分。支持性就业是被公认的能有效促进患者职业康复的模式,但是它更多的运用于院外的精神康复者,在院内应用较少。但是当前院内的患者有强烈的就业需求,而且院内患者社会功能衰退较快,迫切需要通过支持性就业来恢复社会功能。为此,本文着重探讨住院精神障碍患者支持性就业的服务策略。本文采用个案研究法,基于职业成熟度的理论,以社工为研究对象,对住院精神障碍患者的支持性就业服务的可行性、支持性就业服务的策略以及各服务策略之间的关系进行了研究。本研究主要有三个发现,第一,服务对象自身的潜能为开展院内支持性就业服务提供了更多可能性。另外,支持性就业服务拓展了服务对象的就业认知、提升了就业技能、获得了院内的工作机会等成效,有力支持了医院开展支持性就业服务,是可行的。第二,住院精神障碍患者支持性就业的服务策略可以从态度观念、能力素质及行动决策三个方面着手,具体策略包括:(1)帮助服务对象明确态度观念,具体运用教育与参与、示范的介入策略,促使他们达到对自身工作兴趣和期待、对工作的态度和制度有更明确的认知;(2)协助服务对象激发能力素质,具体运用能力的激发、能力的运用的介入策略,推动服务对象发现并发挥自身的能力;(3)推进服务对象的行动决策,具体采用提供行动场景、开展行动调适的介入策略,帮助服务对象在实际的工作场景中不断调适自己从而做出合适的工作选择。第三,支持性就业服务的三个具体策略之间相互影响,态度观念和能力素质方面促进行动决策的推进,行动决策方面又会影响态度观念的改变和能力素质的充实;三个方面的服务具体策略是一种相辅相成的关系。总之,通过对支持性就业服务的策略探索,有助于提升住院精神障碍患者的职业技能,帮助他们恢复社会功能、尽快回归社会。
【Abstract】 In recent years,the prevalence of mental illness has been increasing,and vocational rehabilitation is an important part of promoting the rehabilitation of patients.Supported employment is recognized as a model that can effectively promote the vocational rehabilitation of patients,but it is more commonly used in out-of-hospital mental rehabilitation,and it is rarely used in hospitals.However,patients in the hospital currently have strong employment needs,and the social function of patients in the hospital is declining rapidly.It is urgent to restore social function through supported employment.So this article focuses on the service strategy of supported employment in hospitalized patients with mental disorders.This paper adopts the case study method,based on the theory of career maturity,and takes social workers as the research object,analyses the feasibility of supported employment services for hospitalized mentally ill patients,the strategies of supported employment services and the relationship between the various service strategies.There are three main findings in this study.First,supported employment services have expanded the employment awareness of clients,improved employment skills,and gained job opportunities in the hospital,preventing the decline of social skills of clients and the social network was expanded,those advantages strongly supports the hospital’s supported employment services and is feasible.Second,the service strategy of supported employment for inpatients with mental disorders can be started from three aspects:attitude,ability,and action decision.The specific strategies include:(1)Helping the client to clear the attitude,specifically using education and participation,demonstration intervention strategies,to help clients have a clear understanding of their work interests and expectations,attitudes and systems toward their work;(2)To assist the client to stimulate the ability and quality,we use two services strategie:stimulate the ability and use the ability;(3)To promote the action decision making,we adopt the intervention strategy that provides the action scenario and conducts the action adjustment,helping the service object to adapt itself in the actual work scenario to make the appropriate work choices.Third,these specific strategies of supported employment services interact with each other.Attitudes and abilities promote the promotion of action decision-making.Action decision-making will affect the change of attitudes and the enhancement of ability and quality.Each strategy complements each other.In short,through the strategic exploration of supported employment services,it will help to improve the vocational skills of hospitalized mentally ill patients,help them restore social function,and return to society as soon as possible.
